Phase 8C, Deca Homes, Tacunan, Davao City, do hereby depose and say:

THAT –

1. I am not gainfully employed and have no other means except for the salary earned by
my husband;

2. I am not currently employed with any person, natural or juridical, and still cannot find
any employment;

3. My husband is currently employed with Davao Security and Investigation Agency, Inc.
(DASIA) and his salary is enough for the sustenance and daily expense of our family;
and,

4. I am executing this Affidavit to attest to the truth of the foregoing and for whatever legal
purpose this may serve.
