This matter having been brought before the Court on May 12, 2020, by the Division of
Child Protection and Permanency (the Division), Deputy Attorney General Andrea
Barilli, appearing by phone, and in the presence of:
The Court having reviewed the Division’s report, dated 4/30/20, in evidence, in evidence,
representations made and testimony given if any, and the Court having determined that
the best interest of the child(ren) requires the entry of the within Order; and,
A. Custody
1. The child(ren), , shall continue under the custody, care and supervision of the
Division, with placement as deemed appropriate by the Division, with the Division
being authorized to consent to routine and emergent medical treatment; and with
the Division being authorized to make announced and unannounced visits to the
home(s) of the defendants, and to the home of the caretakers of the children;
2. The child(ren), Yaiana Munoz, Joy Munoz and Shekinah Munoz, shall continue
under the care and supervision of the Division; with the Division being authorized
to make announced and unannounced visits to the home of the defendants;
3. Legal custody of the child(ren), Yaiana Munoz, Joy Munoz and Shekinah
Munoz, shall be
transferred to/ continued with Yashira Colon, Fernando Munoz,
Guadalupe Aarrazola and Edgar Munoz Medrano jointly;
4. Physical custody of the child(ren), Yaiana Munoz, Joy Munoz and Shekinah
Munoz, shall be
transferred to/ continued with Guadalupe Aarrazola and Edgar Munoz
Medrano jointly;
B. Services - Defendants
1. Defendants shall cooperate with the following services arranged by the Division:
a. Defendant Mr. Munoz shall attend evaluation/treatment, as checked below and
comply with all recommendations
psychological, psychiatric evaluation, substance abuse
treatment/random screening at Intervention Specialists.

7. Defendant(s) Ms. Colon and Mr. Munoz shall sign releases of information
insuring the Division access to records and reports of all service providers listed in
this order. Reports of all service providers shall be sent directly to the Division.
C. Services - Children
1. Child(ren) Joy and Shekinah to be participate in individual counseling through
Cooperative Counseling. Yaiana will continue services through CMO;

1. Parent 1/Defendant(s) Mr. Munoz is/are entitled to the following visitation:

5. Defendant(s) Ms. Colon shall notify the Division of any change of address. Upon
receipt of change of address, the Division shall notify all counsel and the court
within 5 days;
7. The court has made a determination regarding the federal Indian Child Welfare
Act (see attached Supplemental Order).
The parents shall refrain from discussing court matters with the children.
The paternal grandparents shall cooperate with individual counseling for the
children and shall be cooperative with a parenting coach.
Cathy Munoz has declined to complete a 5A. If Ms. Munoz changes her mind, she
shall inform the Court.
Supplemental Order is attached to document required findings for additional children
placed out home.
The Court recognizes that at this time, counsel does not have the
opportunity to review this Order, nor does the Court have the ability to
provide all counsel with the Order before they leave the Courtroom
because appearances by counsel in Court are not happening at this time.
The Order will be uploaded to eCourts and all counsel have the obligation
to review the Order and notify the Court of any objections within 5 days. It
is also the obligation of counsel to attempt to provide the Order to their
respective clients and to attempt to discuss this Order to their respective
clients. This provision is included with the understanding that the OPR
office is closed down at this time.

All prior orders not inconsistent with this order shall remain in full force and
effect.
The failure of the Defendant(s) to comply with any provision of this order or
their continuing failure to appear may result in the filing of a complaint by the
Division to terminate the Defendant(s)' parental rights to the child(ren) named
in this complaint. A Termination of Parental Rights would free the child(ren) for
adoption.
Attorneys must review the form of order prior to exiting the courtroom. Failure
to do so waives any objections.
