JUDICIAL AFFIDAVIT OF
ROSELYN ALEGRE

This Judicial Affidavit of ROSELYN ALEGRE is executed to serve as her direct

testimony to prove that:

1. She is the wife of the petitioner, the registered owner of the subject lot;
2. The owner’s copy of said certificate of title was lost;
3. She executed the affidavit loss in relation to the loss said certificate of title;
4. All other matters relevant to this case.

Questions were propounded, by Atty. Siegfried A. Ausa, at his office, located at 3rd Floor
NB Building, Ochoa Ave., Limaha, Butuan City, in Visayan and translated into English which
the affiant fully understands, while answers were given by witness, also in Visayan and
translated into English language.

Answers were given by the witness in full consciousness that she does so under oath and
that she may face criminal liability for false testimony or perjury.

Q1: Please state your name and other personal circumstances.

A1: I am ROSELYN ALEGRE, of legal age, Filipino and with residence address at P-3,
Comagascas, Cabadbaran City.

Q2: What is your participation in the present case?

A2: I am the wife of the petitioner in this case

Q3: Tell us what you want to testify in this case?

 A3: That I am the wife of Almon Alegre. That when we found out that our owner’s copy
of KATIBAYAN NG ORIHINAL NA TITULO Blg. P-1878 was longer being found, I caused
the execution of the affidavit of loss before the Public Attorney’s Office and subsequently caused
its annotation at the back of the Registry of Deeds’ original copy of said certificate of title
denominated as Entry No. 2017000836.
Copy of the Affidavit of Loss which I executed was previously marked in the Judicial
Affidavit of my husband as Exhibit “G”

Q4: Please continue?

A4: That based on records available before the Register of Deeds, there is no registered
claim before said office against the said parcel of land.

Q5: Do you have anything more to say?

A5: None for the moment.
