INTERIM GUIDELINES FOR APPLICATIONS AND COVID-
19 COMPLIANCE CLEARANCE FOR DINE-IN FOOD
OPERATORS
The following Interim Guidelines for Application and Compliance Clearance for dine-in
restaurants and fastfood establishments.

I. GENERAL PROVISIONS

Section 1. Short Title. This interim Guideline shall be known as “ COVID-19 Compliance
Clearance.”

Section 2. Coverage. These interim Guidelines shall cover all new applications for
Compliance Clearance or renewal of Compliance Clearance of all dine-in establishments
except buffet, self-service food business and commercial catering service.

Section 3. Nature and Effectivity. These Guidelines shall be temporary in nature and should
be in effect until further notice by the Local Government Unit of Anda. These Guidelines
may be revised at any time should prevailing circumstance merit.

II.REQUIREMENTS FOR APPLICATION

Section 4. Documentary Requirements for New Applications. With the exception of


buffet, self-service food business and commercial catering service, all Dine-in food operators
applying for Compliance Clearance for the first time are required to submit the following
documentary requirements:
(a) Accomplished Application form which need not to be notarized;
(b) Valid Mayor’s Permit;
(c) Proof of Payment or Official Receipt from Municipal Treasurer’s Office.

Section 5. Documentary Requirements for Renewal Applications and or Reapplication
of Suspended, Revoked Compliance Clearance. With the exception of buffet, self-service
food business and commercial catering service, all Dine-in food operators applying renewal
or reapplication of their Compliance Clearance shall submit the following:
(a) Accomplished Application form which need not to be notarized;
(b) Valid Mayor’s Permit;
(c) Proof of Payment or Official Receipt from Municipal Treasurer’s Office; and
(d) Sworn Statement of Undertaking.

Section 6. Inspection of Premises. As part of certification process and to ensure that it


complies with Minimum Public Health Standards, the applicant shall allow the physical
inspection of its premises.

III. COMPLIANCE CERTIFICATION

Section 7. COVID-19 Compliance Clearance. A Certificate of Compliance Clearance shall


be issued to a Dine-in food operators that:
(a) is found to be compliant with Minimum Public Health Standards, as well as both
basic and minimum standards for the operation of tourism facilities and services; and
(b) has undergone Physical Inspection of its premies.
The Certificate of COVID-19 Compliance Clearance shall be valid for (1) year and shall be
renewable thereafter unless sooner revoked for cause.

IV. OTHER GUIDELINES

Section 8. Templates and Submission. All documentary requirements may be requested


from, and submitted personally or via email to, the Office of the Tourism Department.

attached on the ANNEX A Application Form.

Section 9. Separability Clause. If any portion or provision of this Interim Guideline is


declared null and void or unconstitutional, the other provisions not affected shall continue to
be in force and effect.

Section 10. Effectivity. These Guidelines shall takes effect immediately and shall remain
effective until otherwise superseded, amended, or repealed accordingly.
