MEMORANDUM OF AGREEMENT

Highway, Carig Norte, Tuguegarao City, Cagayan, represented by VALERIANO JESUS V.
LOPEZ, MD, MPH, MHA, CESO IV in his capacity as DIRECTOR IV, hereinafter
referred to as FIRST PARTY;

-and -

CIRILO R. GALINDEZ, MD, MHA, CESO V, Medical Center Chief II, Veterans Regional Hospital,
Bayombong, Nueva Vizcaya herein referred to as the SECOND PARTY;
CIRILO R. GALINDEZ, MD,MHA, CESO V

WITNESSETH

WHEREAS, improving maternal health and reducing neonatal mortality are two of the 8
Medical Center Chief II

Millennium Development Goals (MDGs) that the country aims to achieve. In support of these
goals, the Department of Health (DOH) revisited its programs and protocols and instituted
reforms by means of trainings that develop its human resource capabilities.

WHEREAS, the Department of Health (DOH) 2015 budget for Family Health including
Family Planning, includes provision for support to Maternal, Neonatal and Child Health and
Nutrition;

WHEREAS, the DOH is implementing Administrative Order No. 2008-0029 or the


Strategy to Rapidly Reduce Maternal and Neonatal Mortality;

WHEREAS, the Basic Emergency Obstetric Newborn Care (BEmONC) Skills Training
Course is one activity developed by the DOH aimed at enhancing birth attendants’ skills in
assessing and managing conditions related to pregnancy, childbirth, postpartum, and newborn
care. It is a skills training course composed of both lecture and practicum, where participants
are expected to comply with the required cases prior to issuance of a certificate of training.

WHEREAS, to effect the Local implementation of the said order, particularly Facility-
Based Delivery (FBD), the DOH commits to assists the Local Government Units (LGUs) in
upgrading Basic Emergency Management Obstetric and Newborn Care (BEmONC) health
facilities and to capacitate the Health Workers to deliver integrated package of services that are
critical to improving women and children’s health and preventing maternal, neonatal and child
deaths;

WHEREAS, Veterans Regional Hospital (VRH) is one of the two accredited


CEmONC/BEmONC training hospital in the valley to provide such capability;

WHEREAS, the Department of Health –Regional Office II included in the 2015 Work &
Financial Plan the amount of in pesos SIX HUNDRED NINETY EIGHT THOUSAND ONE
HUNDRED TWENTY TWO & 45/100 (P 698, 122.45) in Geographically Isolated and

disadvantage Area (GIDA);

NOW THEREFORE, in consideration of the foregoing premises, the Parties hereto agree and do
hereby agree on the following conditions:

A. RESPONSIBILITIES:

The First Party shall:

1. Pay Mandatory Fee in the amount of Thirty Thousand Three Hundred Fifty Three
& 15/100 (P 30, 353.15)/participant for the 11-day training period;

2. Provide List of Participants to the Second Party for the said training.

The Second Party shall:

1. Provide the participants necessary skills and knowledge in the Basic Emergency
Obstetric Neonatal Care (BEmONC);

2. Provide the participants with training materials, meals and accommodations during
the training including transportation from quarters/boarding house to training
venue;

3. Coordinate with the First Party as regards administrative matters concerning the
training;

B. AMENDMENTS:

Any amendment, modification, addition, or deletion of any provision of this agreement


shall be agreed upon by the parties in writing.

C. EFFECTIVITY:

This agreement shall take effect upon the signing by authorized representatives of the
respective parties, and shall continue to be valid and binding until terminated by any of
the parties, or automatically terminated pursuant to paragraph 6 of this agreement.
