ENROLLMENT FOR CLINICAL CLERKSHIP

PROCEDURE:

Step I. Kindly accomplish the following immunizations and laboratory exams:

1. Hepa B Antigen Screening
2. Hepa B vaccine
3. Meningococcemia Vaccine
4. Influenza
5. Pneumococcal Vaccine
6. CBC, Urinalysis, Fecalysis (L-NU)
7. Chest X-ray (L-NU)

Submit the results to the Infirmary for Medical Clearance at the Infirmary
(by Schedule)

II. Pay the school fees for the first semester

III. Kindly present the printed ENROLLMENT LOAD to the Admissions at the
Student Lounge to be stamped ACCEPTED and to Registrar to be
Stamped ENROLLED.

IV. Attend the Virtual Orientation (date to be announced later)
