OUTLINE:
1. Payment of Medical Screening fees at UI
MICROFINANCE BANK (N3,000)
2. Check for your X-ray centre on the UI portal
3. Get your X-ray done at your designated centre
4. Check for Screening Centre & Date at the UI
portal
5. Visit your Screening Centre on the assigned date,
for:
a) Medical Screening
b) Medical Registration

Payment for Medical Screening fees

at UI MICROFINANCE BANK

The amount (N3,000) will cover your Medical

Screening at the UHS. Collect a teller and
BRING IT ALONG TO YOUR SCREENING
CENTRE ON YOUR SCREENING DAY.

Check for your X-ray centre on the UI portal

The designated centres:

1. ULTRARAD: UHS (Jaja), University of Ibadan

2. TOTAL CARE: Awosika ave, off Awolowo,

Old Bodija Estate

3. LAB MASTERS: Opposite UI Post Office, near FCMB bank

4. KOMIS: Beside NMA house, Total Garden

5. RAINBOW SCAN: Opp. Oritamefa Baptist Church, Total

X-RAY CENTRE
• Visit your designated Xray centre, PAY for the X-ray and get
it done
• Collect your Chest X-ray FILM & REPORT
• BRING THEM ALONG TO YOUR SCREENING CENTRE
ON YOUR SCREENING DAY

•You can ONLY be screened and registered at your
designated centre and on the date given.

Items to bring to Screening Centre:

•Bank Teller from UI Microfinance Bank (Screening

fee)
•X-ray FILM and REPORT
•Two (2) Passport Photographs
•A pen (to fill your medical forms)

MEDICAL SCREENING PROCEDURE

1. Students sit under canopies in front of sickbay, adequately
spaced.
2. Confirmation of screening dates, names and tellers.
3. Students are given a blank Medical Examination form and a
Psychosocial form to fill in personal details.
4. Vital signs, height, weight and eye test are done on the sick
bay corridor.
5. Go to the lab for Urinalysis, collect result.
6. Go upstairs to sit in the waiting area and arrange documents.
7. Consultation with doctors.
8. Registration with Medical Records (Submit documents)
9. UHS (Jaja) Card is issued to student
10.Student leaves

MEDICAL SCREENING PROCEDURE

In accordance with Covid-19 protocols, the following will be observed:

1. Movement from unit to unit will be done in batches of 5

2. Face masks are compulsory.
3. Social distancing in sitting arrangements and movements
4. Hand washing units

Your Medical Screening & Registration is complete

ONLY after a CASE NOTE has been opened for you
and you’ve been given a Hospital (UHS/JAJA) Card.

ALWAYS brings your JAJA card to the clinic to

receive treatment. You are also advised to store the
number on your mobile devices or commit it to
memory
