HOW TO GET A MEDICAL

CERTIFICATE

Completion of Admission
Requirements for Students
Admitted from 2020 to 2022
I submitted the PEHA Certificate of Compliance. I have enlisted in
my subjects. Why do I need a MEDICAL CERTIFICATE for
ADMISSION?
“Every applicant for admission shall undergo a thorough health examination. No person shall be
admitted to the University who is found by the University Health Service to be suffering from a
dangerous, communicable, contagious, or infectious disease or who is physically unfit to take
courses in any college of the University” (Art. 328, p. 82 Revised UP Code).

As stated in each Pre-enrolment Health Assessment (PEHA) Guide released since 2020, thorough health examination and
diagnostic tests have been deferred. Considering the circumstances of the COVID-19 pandemic, the PEHA Certificate of
Compliance was sufficient for you to enlist in subjects then. Since you will be attending in-person activities on/off-campus
beginning this Academic Year, it is appropriate that you complete the proper PEHA procedure.

Aside from COVID-19, it is important that we, at UP Health Service, have your BASELINE HEALTH INFORMATION and have you
screened for other health conditions that may affect you, your classmates, and other members of the university.
Medical Certificate for Admission
 Issued by a UP Health Service physician
Purpose: completion of UP Diliman admission requirements

 With pre-existing health conditions, immunocompromised, comorbidities?
If YES, obtain MEDICAL CERTIFICATE from family/private doctor allowing you to attend F2F classes or
engage in field activities.

 The Health Declaration Form (HDF) and Mental Health Screening (MHS) Tool are similar to the online PEHA
requirements. To avoid redundancy, UPD Freshmen (2022) will download and print the PEHA Certificate of
Compliance instead.

 Have the Chest X-Ray, Complete Blood Count, and Urinalysis done at your preferred healthcare facility. Please
provide the official reading (TEXT) of the Chest X-ray.

 After September 16, colleges must submit LIST OF STUDENTS (NAME, Student No.) and request for in-
person consultation schedule for their students.

 In 2020-2022 did you: TRANSFER from another UP Campus/School to UP DILIMAN? RETURN from
LOA/AWOL>1 sem? Get ACCEPTED/ADMITTED to GRADUATE studies (finished undergraduate program in any
UP campus/ university)? ---please see NEXT SLIDE.

Step 1: Prepare the necessary documents

Present the original to the UPHS staff; submit photocopy

Step 2: Proceed to UP Health Service
 Arrange and staple your documents following this order:

 Proceed to UP Health Service
 Submit the documents to the UPHS staff for checking
Step 3: Consultation with UPHS physician
 UPHS physician will conduct assessment/ validation
 Issuance of MEDICAL CERTIFICATE (Fit to Enroll)

Note:
 If deemed necessary, the doctor may require diagnostic procedures and/or referral to specialists
before issuance of MEDICAL CERTIFICATE.
 Similarly, doctor may suggest consultation to Psychiatric Services (as needed), based on the student’s
MHS score. This step is not required to secure MC, and may take place whenever the student prefers
to.
 Your MEDICAL CERTIFICATE is VALID FOR 1 year from date issued. Submit to OUR/ your college.
