INSTRUCTIONS

I. Personal Information
This part of the student profile asks for the name and birthdate of the student.
Please follow the format shown on page 4. Please include your middle name, spell out
your birth month, and include your birth year.
II. Psychometric Test Results
This part of the student profile requests for the test results in the following
personality tests: 1) temperament (based from the Four Temperaments), 2) personality
(from the Myerrs-Briggs Personality Test), and 3) intelligence (from the Multiple
Intelligence Theory).
In order to determine the corresponding characteristics, each student must take
the respective tests. Note that the sites are free of charge and that there is no need to
create an account on each site. They have been tested by the editorial board and are
verified to be free of malware. The tests can be taken using smartphones.
The instructions in taking each test are as follows:
1. Temperament
 Take the test at this site: https://openpsychometrics.org/tests/O4TS/
 After taking the test, please take a screenshot or photo of your results as
evidence of taking the test.
To take a screenshot using PC, click the print screen button on
your keyboard, switch to Microsoft Word, and hit ctrl + v (paste) at
the area where the photo is supposed to be located.
If unavailable, please take a photo of your results using your phone
or any other device as long as the image can be embedded in the
document.
 Note the top two temperaments in the form (eg. Melancholic –
Phlegmatic)

2. Personality Test
 Take the test at this site: https://www.16personalities.com/free-personality-
test
 After taking the test, please take a screenshot or photo of your results and
embed it on page 2.
 Insert your top result (only one) on the form.
 3. Intelligence Test
 Take the test at this site: https://www.literacynet.org/mi/assessment/cgi-
bin/results.cgi
 After taking the test, please take a screenshot or photo of your results and
embed it on page 2.
 Insert your top 2 result on the form.

III. Hashtags
This part of the student profile asks you to creatively encapsulate or describe
your MSU life using 2 hashtags. It may it be in terms of what matters to you most, what
event strikes you the most, how you define your MSU life, what people find most distinct
about you, or anything creative. You are free to use any language provided that the
words used will not be offensive. For example, “AsASocialScienceStudent
#BaCalCouldNever #PastilNasad”.

IV. Quote
This part of the student profile requires you to choose your favorite quote from a
teacher with a proper citation. You are free to use any language as long it is a direct
quote from the teacher. For examples, " Kung hindi ka magpagupit, ako magpagupit."

V. Letter
This part of the student profile requires to leave a letter that you would like to say
to your future self. May it be words of encouragement, a past memory, or something
funny you would want to remember. Please answer as concisely as possible in less
than 150 words. Message could be in English, Tagalog or vernacular language.
VI. Submission to Designated Person

After filling up the student profile form, please submit the softcopy to your class
person or to any person in your class assigned to collect the form.

For example,

Please submit the forms on or before the deadline to avoid delays in the
publication of the yearbook. The yearbook staff will not be receiving any more
forms after the deadline.
