AFFIDAVIT OF SELF-ADJUDICATION OF ESTATE OF DECEASED PERSON

I, MARLYN S. DE GUZMAN, married to Mario De Guzman, of legal age, Filipino

citizen, with residence and postal address at Ambiong, La Trinidad, Benguet, Philippines,
after having sworn in accordance with law, depose and say:

That I am the only surviving daughter and heir of the late TERESITA M. SISON who
died intestate on May 10, 2004 at La Trinidad, Benguet;

That said deceased a left real estate property, more particularly described as
follows:

TRANSFER CERTIFICATE OF TITLE NO. T- 46709

A parcel of land (LOt 12-D, of the subdivision plan, Psd-131110-

003839, being a portion of Lot 12, Psd-CAR-000509, situated at
Barangay of Alapang, Municipality of La Trinidad, Province of
Benguet, Island of Luzon. Bounded on the NE., along line 1-2 by
Lot 12-C; of the subdivision plan; on the SE., along line 2-3 by Lot
13, Psd-CAR-000509; on the SW., along line 3-4 by Lot 15, PSd-
CAR-000509; on the NW., along line 4-1 by Lot 12-B of the
Subdivision plan, Psd-131110-003839. Beginning at a point
marked “1” on plan, being S. 12 deg. 32’W., 232.93 m. from BLBM
No. 1, Alapang, La Trinidad, thence; Xxx

Containing an area of TWO HUNDRED EIGHTY

(280) SQUARE METERS more or less; Xxx.

That the said deceased left no debts;

That I am in possession of all the properties enumerated above;

That I hereby requests that this instrument be registered under Act. 3344;

That the provisions of Article 1623 of the New Civil Code and Agrarian Reform Laws
have been complied with.

That pursuant to Rule 74, Section 1 of the Rules of Court, I hereby adjudicate unto
myself the above-described parcel of land including the improvement thereon by means of
this Affidavit of Self-Adjudication and hereby file the same with the Register of Deeds with
the request that the same be made effective without judicial proceedings as prescribed by
the Rules of Court.
