Receptionist: Hello, thank you for registering with us. I just need to ask
you a few questions to complete your patient registration form. Can I just
ask what your title is?

Patient: Yes, my title is Miss.


Receptionist: Thank you, and what is your surname?
Patient: Do you mean my family name?

Receptionist: Yes, that’s right, your family name.


Patient: It’s Al Thani.
Receptionist: Can you spell that for me, please?

Patient: Yes, my surname is Al Thani, that’s A-L and then T-H-A-N-I.


Receptionist: And your first name?
Patient: Laila.

Receptionist: That’s great, thank you Laila. Can I ask if you are single,
married, separated, widowed or divorced?

Patient: I’m single.


Receptionist: And what’s your date of birth, please?
Patient: September 29th, 2002.

Receptionist: September 29th, 2002, OK. And your house number?


Patient: It’s number 24.
Receptionist: Okay, number 24 and what’s the street, please?
Patient: It’s Morden Street in Manchester.
Receptionist: Great, so it’s in the city of Manchester?
Patient: Yes, that’s correct.
Receptionist: And do you have any children Laila?

Patient: No, no children.


Receptionist: Right, and can you tell me your phone number, please?
Patient: Yes, it’s 0-7-9-7-2-9-0-8-2-0-7.

Receptionist: Let me just repeat that. So that’s 0-7-9-7-2-9-0-8-2-0-7 is


that correct?

Patient: Yes, that’s correct.


Receptionist: Good, and who is your next of kin?
Patient: Hmm I don’t understand. What do you mean by next of kin?
Receptionist: Um, who is the person we can contact in case of an
emergency?
Patient: Oh yes, my mother Fatima Al Thani.
Receptionist: That’s great thanks. We’re nearly finished now, but I just
need to ask for your mother’s contact number, please.
Patient: Yes, it’s the same as my number, that’s 0-7-9-7-2-9-0-8-2-0-7.
Receptionist: Okay, and now I just have a few questions about your
medical history. Have you had any immunisations?
Patient: No, none.
Receptionist: And any health-screening tests?
Patient: No, I don’t think so …. Mm no I haven’t had any health-screening
tests.
Receptionist: Okay, good and are you allergic to any medicines?
Patient: Yes, Aspirin.

Receptionist: Did you say Aspirin?


Patient: Yes, that’s right, I’m allergic to Aspirin.
Receptionist: Right, and can you tell me about any operations that you
have had?

Patient: Oh no, I haven’t had any operations.


Receptionist: That’s good, no operations then. And are you taking any
medications?
Patient: None.
Receptionist: Okay, and how would you describe your general health? Is it
excellent, good, fair or poor?
Patient: Oh, I think it’s excellent, I have excellent health.

Receptionist: That’s very good to hear and now the last question. Why do
you want to see the nurse today?

Patient: Well, I keep getting headaches.
