CERTIFICATION

TO WHOM IT MAY CONCERN:

The MEIHAO CORPORATION hereby certifies the following in conformity

with the documentary requirements stated under the Technical Specifications for
the Procurement of various medical equipment for the Maguindanao Provincial
Hospital Datu Piang Annex, the Five (5) Rural Health Units (RHUs), and the Thirty-
Four (34) Barangay Health Stations (BHS), all in Maguindanao, ARMM, to wit:

 That the 95% uptime of it’s product is guaranteed within the

warranty period and that any accumulated downtime in excess of 5%
shall be added to the warranty period.
 That it shall be responsible for the notification, transportation,
delivery, installations and commissioning at no cost to the
government.
 That it has the capability to perform corrective and preventive
maintenance of the product.
 That it’s bid offer is in Philippine Peso to include taxes and duties,
transportation to site, delivery, installation, and testing expenses on
site (hospital).
