appoint and constitute BERLINDA C. CRUIZ, Filipino Citizen, and with address at Torepe
Drive Bosco District Mangagoy, Bislig City, to be my true and lawful attorney-in-fact, for me
and in name, place and stead, to do and perform the following acts and things, name:

1. To act as my representative claim regarding the SSS Benefits for my late husband ;

2. To make, sign execute any documents and other writings of whatever nature or kind with
any and all third persons, concerns or entities that would fulfill her authority granted;

GIVING AND GRANTING unto my said attorney-in-fact full power and authority to do and
perform every act and thing whatsoever requisite or necessary in and about the premises, as fully
to all intents and purposes as I might or could do if personally present and acting in person,
hereby, ratifying and confirming all what my said attorney-in-fact shall do or cause to be done
under and by virtue of these presents.
