AFFIDAVIT OF ACCURACY OF CONTENTS OF

SUBMITTED REGISTRATION DOCUMENTS

I, IBY T. HUAB, of legal age, Filipino, married and resident of #100 Manuel L. Quezon
Street, Siramag, Balatan, Camarines Sur, after having been duly sworn to in accordance with
law, hereby depose and state that:

1. I am one of the partners of RG FASHION GALLERY BY RYAN AND IBY CO.;

2. I uploaded or caused and/or directed the encoding/ uploading/ submission of

the digital copy of the Articles of Partnership of the above partnership using the Company
Registration System (CRS) of the Securities and Exchange Commission (SEC);

3. I confirm that the hard copies of the signed and notarized Articles of
Partnership submitted to and filed with the SEC are true and accurate copies of the electronic
copies encoded/uploaded in the CRS;

4. I am aware that any substantial alteration or material discrepancy

between the encoded/ uploaded/ submitted copies in the CRS and submitted hard copies of the
Articles of Partnership and other attachments, if any, may be tantamount to fraud in the
procurement of the Certificate of Registration or serious misrepresentation which are grounds
for revocation/ cancellation of the partnership’s Certificate of Registration, without prejudice to
other administrative and penal sanctions/ remedies that are available to the SEC under the law.
