AFFIDAVIT OF LOSS

I, HILARION P. OBANIANA III, married, of legal age, Filipino, with address

at Marquez Ext., Brgy. 15 Ilawod East, Legazpi City, after having been duly sworn to in
accordance with law, hereby depose and state that:

1. My wife, Edna R. Obaniana, and I are the registered owners of a parcel of

land situated in Barangay Busay, Daraga, Albay, containing an area of
2,317 square meters, covered by Transfer Certificate of Title (TCT) No.
078-2016001474 issued by the Register of Deeds for the Province of
Albay. A copy of TCT No. 078-2016001474 is attached as Annex “A”;
2. Sometime in July 2019, I discovered that the original copy of the owner’s
duplicate certificate of TCT No. 078-2016001474 was missing;
3. I immediately searched the contents of my file cabinets where I keep
important documents and papers but I could not locate the original copy of
the owner’s duplicate certificate of TCT No. 078-2016001474;
4. After a thorough search on the following days of all the usual places
where I keep all my other important documents, I could not find the
original copy of the owner’s duplicate certificate of TCT No. 078-
2016001474 and believe that it must have been lost or misplaced;
5. Despite diligent efforts to locate the original copy of the owner’s duplicate
certificate of TCT No. 078-2016001474, the same could not be found, as a
result of which, I now consider the same as lost and cannot be recovered;
6. I am executing this Affidavit to attest to the truth of the foregoing and for
whatever legal intents and purpose/s it may serve.
