AFFIDAVIT OF DISCREPANCY

I, ERWIN C. BAÑEZ, of legal age, Filipino, married and a resident of Aridowen, Sta.
Teresita, Cagayan, after having been duly sworn to law, depose and state:

1. That I have been using the name ERWIN C. BAÑEZ as evidenced by my Certificate
of Live Birth issued by the Philippine Statistic Authority;

“A copy of the Certificate of Live Birth issued by the Philippine Statistic Authority is
hereto attached as ANNEX A”.

2. That I used also the said name in all my personal records, transactions and in my
Identification Cards;

3. That I bought a portion of a certain parcel of land from the heirs of Susimo Malazzab
covered by Original Certificate of Title No. P-43454, situated at Masi, Sta. Teresita,
Cagayan;

4. That in the Extrajudicial Settlement of Estate with Absolute Sale made and executed
by and among legal and compulsory heirs of Susimo Malazzab my name was
erroneously written as SHERWIN BAÑES (vendee) instead of ERWIN BAÑEZ;

“A copy of the Extrajudicial Settlement of Estate with Absolute Sale is hereto

attached as ANNEX B”.

5. That the names “ERWIN BAÑEZ” and “SHERWIN BAÑES” refers to the name of
one and the same person, the Affiant;

6. That I am executing this Affidavit to attest the truth of the foregoing statements and
for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 10th day of August, 2020 at
Tuguegarao, City, Philippines.
