1. Scope
This procedure describes the steps and requirements to be verified for the
implementation and the maintaining of a supervision scheme.
Supervision activities are essential to ensure an effective quality assurance
system for essentially third party agencies where integrity and reliability are
essential values to be protected.
For any inspection agency a Supervision Scheme of its services is essential
to:
 assess the accuracy of inspection results,
 maintain a high standard outputs,
 ensure requirements of codes, standards and regulation are fulfilled
 identify inspectors’ capabilities,
 plan training / mentoring sessions, and
 ensure customer satisfaction
This procedure sets the minimum requirements to ensure an acceptable
output from any inspection performance by detailing supervision activities at
three stages:
(1) Before starting the inspection/testing activities
(2) During the inspection/testing activities
(3) After accomplishment of inspection/testing activities

3. Terminology and Abbreviations
Supervisor: Is the person assigned by the technical lead to conduct the
supervision activities on the inspector. It shall be of a level higher than the
level of the inspector.
Inspector: The person that will be supervised and he is conducting the
inspection activities as per the supervision planning.
Supervision Scheme: The planned supervision activities to be
implemented/revised periodically to allow well-controlled inspection activities.
The objectives of such scheme is to monitor the capability of the inspectors
and to plan training activities on a more effective way.
4. Responsibilities
4.1. Top management
As per FALCON Quality Manual some of the management commitment are in
a close relation with developing and maintaining such supervision scheme
such as:
(1) Develop and Maintain the competence of all personnel via a supervision
process and active training
(2) Satisfy all applicable legal and normative requirements
(3) Commitment to high standard quality by the means of risk prevention
culture.
(4) Produce reliable results and clear supports respecting deadlines
Top management is responsible for the implementation of this supervision
scheme and the provision of necessary resources for its correct application
and maintaining.
4.2. Human Resource Responsible
Human Resource Responsible should maintain and update a documented
information of the level of qualification of all inspectors.
Criteria of classification should be clearly defined with the technical
responsible. Such criteria may include the condition at which an inspector can
be a level 1, 2 or 3 inspectors, the condition of a decategorization ect…
These criteria should be revised at regular intervals.
A planning of supervision should be prepared and maintained at regular
intervals.
4.3. Technical Lead
Technical lead is the first responsible of the inspector to be supervised.
He is responsible to the realization of the planned supervision and to inform
concerned parties about the details of such interventions. Concerned parties
are:
(1) Human resource responsible
(2) Top management
(3) Supervisor
(4) Inspector
(5) Client
The technical lead shall assign a mission that matches the inspector (deemed
qualified to perform it).
Technical lead is responsible to communicate the supervision result to the
inspector in a constructive manner to build self-awareness and to ensure
continuous improvement of the provided services.
4.4. Supervisors
Supervisors assigned the task of NDT Supervision shall be of a recognized
qualification in the specified technique. A recognized qualification may be a
granting NDT qualification by a recognized third party such as COFREND and
CSWIP, COTEND, or any recognized inspection certificate in a specified field
such as API for pressure vessels, piping and storage tanks or AWS for
welding…
As a general rule the supervisor shall be at a level higher than the inspector to
be supervised. The level of qualification is as per paragraph 4.2.
A Level 2 inspector for example can only be supervised by a level 3
supervisor. A level 1 inspector can be supervised by a level 2 or 3 supervisor.
Supervisors are responsible to conduct the supervision itself and is asked to
provide his feed back to human resource and the technical lead as soon as
possible.
4.5. Inspectors
Inspectors are those personnel that perform the required inspection/Testing
techniques and for the purpose of this procedure are those who will be
supervised.
Inspectors are responsible for:
(1) The well preparation of the mission
(2) Communication with client
(3) Respecting inspection codes, standards and best practices
(4) Respecting client requirements if any
(5) Respecting safety requirements
(6) Performing the inspection/testing
(7) Assessing the client with recommendations
(8) Reporting
During the supervision activities, inspectors should demonstrate the
knowledge of their responsibilities and their implication on providing a reliable
outcome. Supervisor will assign a note on each of these aspects and can if
necessary provide more details to describe some deficiencies if any.
An Inspector shall be assigned a mission that matches his profile and shall be
deemed qualified to perform it.
The inspector should be familiar with quality processes interactions that affects
the realization of its mission, this is incudes essentially support processes.
Inspectors shall be kept informed by his technical lead about the result of his
supervision. It is essential to build self-awareness which helps the inspector to
progress.
5. Supervision Scheme
5.1. Planning
A list of all inspectors by level of qualification is maintained and annually a
planning of supervision shall be updated or when deemed necessary.
Supervision planning of inspectors shall take into consideration as far as
possible the principal of multidisciplinarity. An inspector with a low level of
qualification in a discipline shall be supervised as soon as possible to allow
corrective actions to be taken and to allow him to get a high level as soon as
possible.
Furthermore, any person who has not performed inspection, examination, and
testing activities in his qualified area for a period of one year shall be re-
evaluated.
Well planned supervision with accurate feedback can help the decision
makers to determine the capabilities of a candidate for an eventual
certification.
After approval, planning shall be shared with top management and technical
leads for information and actions respectively.
5.2. Fixing Effective date / Supervisor
The technical lead is responsible to respect the supervision planning as
described above.
Assignment of a supervisor shall be as per paragraph 4.4.
The technical lead should inform the inspector and the supervisor about the
details of the supervision mission (Date, Location, Client, technical details…).
Client shall be advised about the nature of this intervention.
Client details / contact should be shared with the inspector / supervisor to
allow any further correspondence/clarification if needed.
5.3. Supervision before starting the inspection/testing activities
The mission starts from the moment the inspector begin its preparation.
The supervisor is asked to observe the attitude of the inspector at this stage
without interfering nor stressing him. Check points at this stage includes:
(1) Had the inspector the required qualification to perform the intended
inspection?
(2) Had the inspector contacted the client to confirm his intervention and ask
for further details (specific requirements, client expectations…)
(3) Had the inspector collected the required procedures if any and review it?
(4) Is the inspector aware about any regulatory/standard requirements that are
related to its intervention?
(5) Had the inspector checked the technical details related to its mission: is
these details available, if not what he is planning to do?
(6) Had the inspector got its mission order?
(7) Had the inspector got any required inspection equipment and checked it
before signing the exit split (Calibration, Not broken, Tested, calibration
certificates)?
(8) Had the inspector checked its Personal protection for availability?
(9) Does the inspector know what report he will be producing after the
mission?
(10) Does the inspector get a time sheet for invoicing and does he aware
about any commercial requirements?
(11) Does the inspector a mean of transport and is it reliable?
Each of these above points shall be appreciated by the supervisor and a note
shall be given to access the inspector attitude.
5.4. Supervision during the inspection/testing activities
When arriving at client facility the following points shall be supervised:
(1) Is the inspector wearing all required Personnel Protection? Is there any
special protection requirements that should be taken into consideration?
(2) Had the inspector contacted the first responsible of the inspection activities
from the client side? How it was his approach / first impression?
(3) Did the inspector clarify his intervention with the client? Did he asked for
additional information that he consider essential to the performance of the
inspection?
(4) Did the inspector inspect the area of working for potential risks and
hazard? Is the area safe for the intended activities and for personnel
involved?
(5) Had the inspector checked that surfaces are properly prepared and clean,
before and after testing?
(6) Had the inspector identified the equipment subject to the inspection and
delimitated/recorded the extent of his intervention on this equipment.
(7) Had he indicated any zone which cannot be examined or is outside the
scope of the test and advised the client?
(8) Had the inspector calibrated any inspection equipment before use? The
sensitivity of equipment settings can deviate in time, so did the inspector
checked the accuracy of any used inspection equipment from time to time?
(9) Had the inspector recorded the following information which are required for
reporting:
a. references of any consumables used and their use-by dates,
b. references of equipment for testing and the methods chosen,
c. the zones undergoing testing,
d. the limits of any zone which cannot be examined or is outside the
scope of the test.
(10) Had the inspector means of recording the tests performed? Is it
reported suitably?
(11) Had the inspector located every Reading/Imperfections/Defects, in
order to allow later tests to be carried out, and ensure that readings may
be assessed against the acceptability criteria as applicable?
(12) Is the inspector aware about any standard/Regular requirements during
the intervention?
(13) Can the inspector use the inspection equipment correctly? Is the
definition of readings correct?
(14) Has the inspector reported any deviations from the specified
procedures and why?
(15) Has the inspector assessed the result of the inspection? Is it well
communicated to client? Had he stated a clear statement of conformity or
non-conformity
(16) Is the interpretation of inspection result correct?
(17) Had the inspector provided a flash report to the client? By what mean?
Is it consistent with the real state?
(18) Had the inspector signed off the timesheet correctly?
(19) How did the inspector finish the mission? How it was his attitude with
the client?
5.5. Supervision after accomplishment of inspection/testing activities
The mission is not yet finished. Now the supervisor shall allow the inspector to
prepare the related report and after that he should evaluates the following:
(1) identification of manufacturer of equipment tested,
(2) identification of each object examined,
(3) reference to application documents and the validated testing procedure,
comprising acceptability
(4) identification of examination devices used, and more specifically
references, brands and identification numbers, as well as environmental
conditions for their use (e.g. temperatures),
(5) testing instrument settings and identification of reference blocks used if
any,
(6) classification and location of readings,
(7) conclusion of testing (consistency of readings),
(8) any deviation from the procedure,
(9) Is the report well verified, approved and signed by required personnel?
(10) If a software is used for assessment purposes, is it accurate? Could it
lead to incorrect results?
5.6. Supervision outcomes
The supervisor shall issue a supervision report with detailed observations. The
feedback shall be provided to human resource and the technical lead as soon
as possible by means of a report or a check sheet as defined by human
resource manager.
Feedback is used to update the qualification matrix and to update the
qualification level of the inspector. Corrective actions can be planned also
such as new training or a refreshing training or even procedures’ updates in
some cases.
