ELECTROCARDIOGRAM MONITORING(ECG)
Definition: It is a graphic record of electrical currents produced by activity of the heart., to determine
cardiac rhythm and pattern variation, which may reveal pathologic processes
Purpose: Useful in Diagnosis of
Cardiac Arrhythmias
Myocardial ischemia and infarction
Pericarditis
Chamber hypertrophy
Electrolyte disturbances
Drug effects and toxicity

1. Verify the doctors order
2. Gather equipment
-Electrocardiograph machine -ECG Strips
-10 Electrodes - Tissue Paper
-Conducting Jell
3. Wash hands
4. Monitor the BP of the patient
5. Explain procedure to patient
6. Change the dress of the patient to gown
7. Remove all metal items from the patient’s body (e.g. earrings,
coins, belt, necklaces, and the like.)
8. Clean areas thoroughly on patient’s chest wire electrodes will be
attached.
9. Attach the limb electrodes to monitor cable using appropriate
colored wires. Attach to the patient extremities
RA - RED
LA - YELLOW
RL - BLACK
LL - GREEN
10. •Apply electrode paste and attach electrodes to the patient chest:
Females: Choose a site as close to standard position as possible.
• V1—4th intercostal space (ICS) at right sterna border.
• V2—4th ICS at left sternal border.
• V3—Midway between V2 and V4.
• V4—5th ICS at midclavicular line.
• V5—Left anterior axillary line at level of V4 horizontally.
• V6—Left midaxillary line at level of V4 horizontally
11. Observe pattern to determine clarity of image of oscilloscope
12. Run ECG strip
13. Read the Pulse Rate
14. Remove the chest electrodes from the chest and extremities
15. Wipe/clean the gel from the chest with a tissue paper
16. Write the name of the patient and the BP reading on the ECG
strip (on the upper left corner)
17. Arrange and Attach the strip to the Chart
18. Refer to the doctor for the interpretation.
