PRACTICUM WAIVER

Dear Sir/Madam:

This is to respectfully inform your good office that permission is hereby granted to my child, Goemer
A.Resurreccion to participate in the On-the-Job Training Program that is a requirement in the course
Bachelor of Science in Business Administration, under the Commission in Higher Education.

I understand that the above-mentioned knowledge and skills training will cover 300 training hours, which
will be conducted in Jamaica Realty & Marketing Corporation, 380 Don Manuel Subd., Bulilan Sur, Pila,
Laguna, from November 18,2019 to March 2020

I am aware that the program coordinator will monitor the performance of the student and conduct routine
visitation, and that the immediate supervisor within the company will take all the necessary precautions to
keep the student safe.

However, if despite all efforts taken, untoward incidents beyond their control occur, we will not hold Union
College, Program coordinator and the company stated above where my son/daughter took On-the-Job
Training liable.
