TRAINING AGREEMENT AND LIABILITY WAIVER

I, Ivern Fantilanan, a student of Laguna College with postal address at Blk 13, Lot
5 Southwynd Residence, Brgy.Sta.Monica San Pablo City, Laguna - hereby voluntarily
agree to undergo On-the-Job Training which is an academic requisite for Graduation at
the said school for a period of 600 hours as prescribed by the (CHED) Commission on
Higher Education, under the following terms and conditions:

a. That I shall abide by all corporation’s rule and regulations and shall comply with
those imposed for the training, otherwise, I shall be excluded from further
participation;

b. That there is no employer-employee relationship between me and South Quest

Company;

c. That I shall exercise care and diligence in any task assigned to me;

d. That I renounce and waive my claim against South Quest for any injury that I may
claim sustain or loss, suffer, personal or pecuniary, in the performance of duties or
functions while under training.

e. That I shall be made answerable for any and all liabilities for damage to property
or injury to third person which be occasioned by my intentional or negligent acts
while in the course of my training.

f. That if I opt to have in-person training I understand the risks present in the current
situation and that I will take all necessary precautions and observe health
protocols advised by the school and the company. I also understand when I am
asked to undergo isolation or quarantine. I will not take this against the company
nor the school as this may have bearing on the suspension of training hours,
duration of training, and probable unavailability of slot.
