I have the honor to apply for my transfer from Matmad Integrated School to Anak Integrated School or any
nearby Junior High School to where I am residing Barangay Dissimungal, Nagtipunan Quirino for the following
reason to wit:

1. I have been married for four (4) years but still childless because of daily tedious travel from home at my
station;
2. I am not physically fit to serve in my current station due to polycystic ovaries bilateral that triggered my
situation because of my irregular check-up and,
3. I am a wife of a Police Officer presently assigned at Nagtipunan Municipal Police Station, Ponggo,
Nagtipunan, Quirino moreover there is a presence of New People’s Army at my station;
4. I’ve been teaching at Matmad Integrated School for almost 3 years.

It would be a great favor if you consider my request regarding this matter.
