Due to the distance of my working station from my residence in Caridad, Dumingag, Zamboanga del Sur,
it is my utmost intention to request from your good office an assistance for my Application for Transfer through
swapping from Dangolaan Central Elementary School, Dangolaan, Sultan Naga Dimaporo, Lanao del Norte with
the person of Mrs. Leonida Pulgo Yabo from Dapiwak Elementary School, Dapiwak, Dumingag, Zamboanga del

 SWAPPING AGREEMENT

KNOW ALL THESE MEN BY THESE PRESENTS:

This Swapping Agreement made and executed by and between; RAYMOND

CHRISTOPHER POLINAR ONG, Filipino, of legal age, single, presently assigned at
Dangolaan Central Elementary School, Dangolaan, Sultan Naga Dimaporo, Lanao del
Norte, Region X, Philippines, hereinafter called the FIRST PARTY and LEONIDA
PULGO YABO, Filipino, of legal age, married, presently assigned at Dapiwak
Elementary School, Dapiwak, Dumingag, Zamboanga del Sur, Region IX, Philippines,
hereinafter called the SECOND PARTY.

WITNESSETH

That the FIRST PARTY is presently assigned at Dangolaan Central Elementary

School, Dangolaan, Sultan Naga Dimaporo, Lanao del Norte and the SECOND
PARTY is presently assigned at Dapiwak Elementary School, Dapiwak, Dumingag,
Zamboanga del Sur

That the FIRST PARTY is willing to be assigned/swapped with LEONIDA

PULGO YABO at Dapiwak Elementary School, Dapiwak, Dumingag, Zamboanga del
Sur; while

The SECOND PARTY presently assigned at Dapiwak Elementary School,

Dapiwak, Dumingag, Zamboanga del Sur is likewise willing to be assigned/swapped
with RAYMOND CHRISTOPHER POLINAR ONG at Dangolaan Central Elementary
School, Dangolaan, Sultan Naga Dimaporo, Lanao del Norte.

That both parties hereto have mutually agreed to exchange/swap their respective
assignments and duties of the above mentioned school of assignments to better serve
the government and family subject to the approval of their Schools Division
Superintendent, District Supervisors and respective school principals.

WHEREFORE, the said FIRST PARTY does hereby TRANSFER/WAIVE his

SCHOOL DUTY OF ASSIGNMENT to the SECOND PARTY at Dangolaan Central
Elementary School, Dangolaan, Sultan Naga Dimaporo, Lanao del Norte; and
likewise, the SECOND PARTY, in turn, does hereby TRANSFER/WAIVE her
SCHOOL DUTY OF ASSIGNMENT to the FIRST PARTY at Dapiwak Elementary
School, Dapiwak, Dumingag, Zamboanga del Sur.

That this agreement is voluntary entered by both parties, without being coerced
nor intimated by any other person whatsoever.
 IN WITNESS WHEREOF, we have hereunto set and affixed our signature this
