    I am Injell Pyar R. Suni, 21 years old, presently residing at Brgy. #10, Lubigan, Badoc,
    Ilocos Norte and a fifth-year BS Ceramic Engineering Student. I enrolled my undergraduate
    thesis (CERE 200) this 1st semester SY 2020-2021 as residency status. My study requires
    experimental data and therefore I need to go to school and do the laboratory works.

    At the present situation of Ilocos Norte under the Modified General Community
    Quarantine, may I humbly request permission to conduct the laboratory activities at the at
    the Department of Materials Science and Engineering, College of Engineering.
