In view of recent outbreak of COVID-19, and to take strict measures against

spread of Corona, it is required that all agencies closely monitor the health
status of their staff.
You are requested to forward following information on daily basis to SPG by
0900 hrs.

1. Whether any of your Staff/worker or family member(s) living with him are is
having any symptoms such as Cold, Fever, Cough or suffering from any other
ailment etc. If yes, give details.

2. Whether any of your Staff/Worker or their family members have visited or

been admitted to any Hospital/Dispensary/Clinic during the last 14 days? If yes,
kindly give details.

down area as declared by local authority? If yes, kindly give details.
