On the evening of Sunday, September 6, at around 8:50 pm the bar area at Zipps
Sports Grill was again packed with patrons sitting as well as standing and
mingling. Restaurant staff made no effort to physically separate patrons or
require them to be seated. See photos:

As noted in Attachment A, 414 Pub & Pizza advertises karaoke and patrons were
again participating in karaoke in the restaurant at around 9:15 pm and 9:35 pm on
the evening of Sunday, September 6. Patrons were also seen standing and
mingling without masks. Restaurant staff made no effort to prevent patrons from
participating in karaoke in the restaurant. See photos:
Loco Patron
222 S Mill Ave #115, Tempe, Arizona 85281
On the evening of Sunday, September 6, there were fewer patrons at Loco Patron
but they were not avoiding close contact. The bar top was full. Again restaurant
staff made no effort to physically separate patrons or require them to be seated.

On the evening of Sunday, September 6, around 7:30 pm all the tables outside
Kung Fu Tea were occupied and not properly distanced. See photos:
