This determination notifies you, that you are financially eligible for Pandemic Unemployment Assistance
(PUA) benefits pursuant to 20 Code of Federal Regulations (CFR), Part 625. Provided you meet all program
deadlines and eligibility requirements during the week(s) claimed, you are eligible for a weekly benefit
amount (WBA) of $158.00.

A determination notifying you of monetary eligibility does not guarantee payment of unemployment
insurance benefits. You must meet ALL other eligibility requirements of the law in order to receive
payment.

This weekly benefit amount is computed for a week of total unemployment under the applicable provisions
of this state’s unemployment compensation law and is based on your earnings during your base period.
