Notice of Monetary Determination

ELIJAH SANE,

Your claim for Pandemic Unemployment Assistance was filed on August 1, 2021. Information about your
monetary eligibility is explained below. You should keep this letter for reference throughout the duration of your
claim.

Decision

You meet the monetary eligibility requirements to establish a claim for Pandemic Unemployment Assistance
benefits. You are entitled to receive benefits as long as you are not disqualified or held ineligible for other
reasons.

Claim Summary
• You may receive up to $263.00 each week you are eligible for benefits.
• You are eligible for up to 79 weeks of benefits.
• Your maximum benefit amount is $20,777.00.
• You are entitled to file for benefits beginning from December 6, 2020, if otherwise eligible.
• Unless federal law changes, the program will end with the week ending September 4, 2021.

The American Rescue Plan Act of 2021 (ARPA) increased the maximum amount of benefits available under the
PUA program from 50 weeks to 79 weeks. If you were eligible for weeks of extended benefits due to the high
unemployment period, those weeks will be included in the total number of eligible weeks above. If you exhausted
your PUA benefits prior to the week ending March 13, 2021, you may be eligible for additional benefits beginning
with the week ending March 20, 2021. You do not need to apply to receive the additional benefits, but you must
continue to file your claims each week for the PUA program. These additional 29 weeks of PUA benefits may only
be collected for weeks ending on or after March 14, 2021. Any weeks in which you received regular
unemployment compensation (UC) or Extended Benefits (EB) will be subtracted from this amount.

Starting with week ending January 2, 2021 and ending with the week ending September 4, 2021, you will receive
a $300 supplemental payment from the Federal Pandemic Unemployment Compensation (FPUC) program for
any week that you are eligible for PUA.

The maximum benefit amount shown above does not include any Federal Pandemic Unemployment
Compensation (FPUC) that you are potentially eligible for. You do not need to contact the agency or submit a
separate application to be considered eligible for the additional $300 FPUC weekly benefit amount. It will
be automatically applied to your payment for qualifying weeks and is separate from your Pandemic
Unemployment Assistance weekly benefit amount.

You reported Income for 2019 of $15,000.00.

Quarter of Wages Amount

1st Quarter 2019 $3,750.00
2nd Quarter 2019 $3,750.00
3rd Quarter 2019 $3,750.00
4th Quarter 2019 $3,750.00

Computation of your Weekly Benefit Amount: Hawaii Revised Statues 383-22, states in part… (1) An individual’s
weekly benefit amount is equal to the high quarter wages divided by 21, but not more than the current maximum
weekly benefit amount. (2) If your earnings were $22,092 or less, you would be eligible for the minimum PUA
benefit of $263.

If your net income for 2019 exceeded $22,092, you may qualify for more than the minimum weekly benefit
amount (WBA) of $263. You may request a redetermination of your benefit amount any time during the Pandemic
Assistance Period which ends March 13, 2021. You will have 21 days to submit income verification
documentation following your request for redetermination.

Weekly Certifications

You must file weekly claim certifications every week to continue receiving benefits. Failure to certify each week
will result in non-payment for that week.

Online: To certify online, visit https://pua.hawaii.gov/ and log in to your account. Your online account is
accessible twenty-four hours a day and seven days a week.

If you disagree with this determination, you have the right to file an appeal. Your appeal must be received within

appeals officer if “good cause” is established. To file an appeal online, go to https://pua.hawaii.gov/ and log in to

appeal and this letter to the Employment Security Appeals Referees Office (ESARO), 830 Punchbowl St. Room

determination is reversed, you will not receive benefit payments for the weeks that you did not file your
weekly claim certification.
