Dedication

This Project is dedicated to our Parents and Instructor who were always there to support us

throughout this academic course.

We would like to thank Almighty Allah who blessed us with such wisdom and intellect that we

are able to cope with the demands of this project and course.

Special thanks to our instructor Sir Sikander Aziz who always motivated us to work hard

throughout the session not only in his respective course but in all courses. It has really been an

honor to study under his supervision.

This Project would not have been possible without any single member of this group. Well done

all of us.
Data Elements
There could be hundreds of data elements but after filtering and relating process 40 were
selected. This list of data elements is prepared to design a comprehensive MIS of a hospital and
there is margin for upgrading or improving it since it is a technical but a flexible MIS.

Following is the list of data elements.

2. Patient First Name

3. Patient Last Name

4. Patient Full Name

5. Patient Address/City

6. Patient State

7. Patient DOB

8. Appointment/Admit Date

Arranging Data elements into logical tables
While arranging the data elements they were already listed in an order so that there is no

difficulty in arranging them in tables and further those tables can be related easily.

1. Patient table was designed to be the basic or first information table which would be necessary

to process any patient’s treatment while keeping its full record. Here Patient ID would be the

Key field which is the most important key in whole MIS because it will interconnect all other

entities and would make it easier for the end user to collect or generate records of a single

patient.

2. After collecting the first information patient will be referred to a department relative to
his/her condition. Here according to the condition and hospital schedules a patient will be
granted an appointment to meet a specialist.

Appointment
Appointment/Admit Date
Appointment/Admit Time
Appointment/Admit Duration To differentiate between customers

3. The Doctor table is designed to make the hospital management more systematic or in easy
words this table will make sure that all the patients are equally divided to all doctors so that
one doctor is not taking all the burden but it is shared, moreover patients are going to get a
doctor relative to their condition. Most importantly here doctor will prescribe patient with
some drugs and recommend some tests.

Doctor If a patient comes back with the same

Doctor ID issue or a different one then the same
Doctor Name doctor treats that patient if not then at
Prescription ID least patient has a previous record for

4. This table is important for not only keeping record of patients but making sure that patients
are provided by effective and efficient services. All further provided services are going to
count on prescription.

5. If the patient is admitted then automatically a nurse and a room would be issued to the
patient. During the duty hours nurse would take care of the patients regardless of the visitors.

Nurse
Nurse ID
Nurse would be aware that what

would be directed that which room
to be allotted to the patient
6. Here patient’s condition will be monitored by the doctor and the nurse and updated on patient
profile. Most importantly this table will also determine the end bill because after the
admission form all patient expenses are going to be connected with the room.

Room

Check in Date/Time To check the availability of rooms
Discharge Date/Time and provide rooms to other patients

7. Here all the prescribed tests are going to be recorded and patient’s health, diet and drugs are
going to be determined and updated.

will designed and it will be monitored by the nurse.

9. After some time or just before discharging the customer, an invoice will be generated against
the patient ID and this will be done by the help of the record where all the services and
treatments provided to the patient are mentioned.
