4 Healthy Decision Assignment (20 Points)

Instructions: For this graded assignment, you will evaluate the effectiveness of a health-related decision you
have made in the last month.
Choose an unhealthy decision you have made recently that could affect your overall well-being, and then
evaluate that decision based on the six step process you learned about in Lesson 1.4. You may concentrate
on your eating or sleeping habits, risky behavior choices, personal safety decisions, or anything else you can
think of that directly affects your health. The purpose of this assignment is to get you to evaluate and improve
your choices in order to maintain optimum health.
Record your answers in the space provided:
Step 1: Describe the situation that required a decision. Then, state the unhealthy decision you made, and
explain the decision making style you demonstrated when you made that decision. (3 points)

Step 2: List the possible decisions you might have made in that situation, including the one you made. You
must list at least THREE choices, including the decision you previously made. (3 points)
