Fellow Applicant Template

This Fellow application template has been created to support individuals in structuring their
application to fully address all the criteria for Descriptor 2 of The UK Professional Standards
Framework (UKPSF).

The evidence requirements for Fellow are set out in the Fellow guidance document for
applicants (available through downloading your Fellow Pack) which should be read prior to
writing your application in this template.

This application template should be completed in accordance the above guidance document
and the following:

 The UK Professional Standards Framework (UKPSF), Descriptor 2


 Fellow overview on HEA website
 Fellow referee guidance notes for applicants

This template has been designed so that you add text under the sub-headings provided across
your application. No appendices or additional documents are required as part of the
application (other than your two referees).

Once completed, this template can be sent to your referees to corroborate your application and
then used to complete your online application via your HEA MyAcademy.

Evidencing A1: Design and plan learning activities and/or programmes of study
