    Between Mr.Kartik Vishnoi & Dr.Arun Prakash Dwivedi

    I Kartik Vishnoi S/o Shree Anil Kumar Vishnoi R/o C-490, MIG Barra-6, Kanpur AND Dr. Arun
    Prakash Dwivedi Dhanwantari Hospital Kanpur agreed that-

    1. That I Kartik Vishnoi bonded to serve for 4 years as assistant for medical job to Dr. Arun
    Prakash Dwivedi.

    2. At the starting of job I got a salary of Rs.8000.00 per month and on completion of three
    moths I got Salary of Rs.15000.00 per month.

    3. If I got a government job then I can leave the job.

    4. If I leave job other than Govt Job before 4 years I am bonded to pay Rs.50000.00

    5. If there is 5 cases in a day outside the Hospital then salary is not variable.

    6. if cases exceed more than then 5 then extra payment has to be made by Hospital.
