I, the undersigned, Praveen Singh S/o Shivpal Singh aged 38 years, Hindu by religion, presently &
permanently residing at H.No.126,Saraymeeta Panki, Kanpur, do hereby solemnly affirm and declare
on oath that:-

1. I am the deponent herein and a citizen of India.

2. I was born on 04/04/1982 at Kanpur Uttar Pradesh India.
3. That in the PAN Card bears the name of mine as Praveen Chandel.
4. That by mistake my name has been mentioned as Praveen Chandel instead of Praveen Singh in the
said document.
5. This is also to categorically state that my correct name is Praveen Singh which is mentioned
in my Aadhar Card.
6. That I therefore request the concerned authorities to may please update & consider my name
as Praveen Praveen Singh as per Aadhar Card.
7. That I am enclosing a self attested copy of Aadhar as proof of my correct name.
8. That my above statements are true and correct.
