1. A request letter from either Fiji Citizen Parent(s)

2. A support letter from the guardian if the child is not

residing with his/her parents in Fiji.
3. Medical report for applicant conducted within three (3) months or less
from the date of application
4. Certified copy of the child/children’s passport (Bio-data page)

5. Two (2) recent passport size photographs of child/children

6. Certified copy of child/children’s Birth Certificate

7. Certified copy of either parents’ Fiji Passport

8. Copy of previous exemption Permit

9. Fees NON APPLICABLE
