MEDICAL EXAM
Students should secure Medical Certificate from their College/University school Using RED clearbook, please compile below documents and submit it to
physician. The certification must bear remarks “Physically fit to join the Student DSII on or before November 13, 2015
Cultural Exchange and Travel Program USA Season 2016”.
1. Transcript of records indicating 2nd Semester enrolled subjects.

US EMPLOYER’S INTERVIEW
Bring your original receipts on the day of the Interview. DSII will issue you an
Official Receipt for payments made. Exact Date, Time and Venue shall be
announced soon. You need to submit the following:
1. Photocopy of passport
2. School Certificate with expected year of graduation (2 originals) APPLY J1 VISA AT US EMBASSY MANILA
3. Resume with 2x2 picture
