7.3.1 Time

The exposure received by an individual working an area where there is particular

radiation intensity is directly proportional to the time spent in that area. Thus the less
time spent in the radiation are then the smaller the exposure. The maximum allowable
times for source recovery are shown in Appendix A.

LESS TIME = LESS EXPOSURE

7.3.2 Distance

The radiation intensity from a source inversely proportional to the square of the
distance from the source. This is known as the inverse square law and can be
expressed generally as:-

Safe distance = Output X source activity

Maximum permissible dose rate (µSv/hr).
Where:-
Safe distance is in meters
Output is in µSv /hour at 1 meter
- 4800 µSv /hour for 1 ci Ir 192
13000 µSv /hour for 1 ci Co 60
Source activity is in curies
Maximum permissible dose rate in mSv /hour
eg. The safe distance from a 25 ci open Ir 192 source is,
With a maximum permissible dose rate of 7.5 µSv /hr

Safe distance = 4800 x 25

7.5
= 126.5 meters

Using Tungsten Collimator

Safe distance = 4800 x 25

7.5 x 40
= 20 meters
Note: Attenuation provided by Tungsten Collimator -- B
For Shielding is 1 TVT + 2HVT, B = 40
When working in a radiation area always maximise the distance from the Source.

GREATER DISTANCE = LESS EXPOSURE

Shielding can greatly reduce radiation exposure and any available shielding should
be used to advantage. Collimators should be used whenever possible.
It may for instance be possible to work behind a concrete structure which would
provide shielding from the radiation. In field application many items are available for
use as shielding. A useful concept for estimating shielding efficiency is the half / tenth
value thickness which is defined as the thickness of shielding material required to
reduce the radiation to one half / tenth of its original value. A table of half /tenth value

10 EMERGENCY PROCEDURES

10.1 Introduction

Emergency conditions are those in which the source cannot be returned to the
shielded position by normal means. The most important factor in an emergency is to
recognize that an emergency exists. This can only be accomplished by the proper use
of a survey meter which can give valuable information as to the nature of the
emergency, (e.g. whether the source is jammed in the collimator, guide tube or exit
port). Each emergency should be treated individually and the entire operation thought
through from beginning to end before any action other than erecting barriers and
informing the correct authorities, is taken. Always use as much shielding as possible
without hampering the emergency operation. In accomplishing emergency
procedures it may be necessary to work in high radiation intensities. Determine the
amount of time necessary to be in these areas and compute the expected exposure
before performing the operation. Only thorough careful planning and application of the
principles of time, distance and shielding can emergencies be dealt with safely.

Within the next paragraphs the instruction are given on how to shutdown in case of
emergencies. If none of this instruction apply and/ or give the desire effect, please
contact the emergency response phone as it must be written on the supervisor
sign

Emergency action is the overall responsibility of the MME. However, the initial steps
for the rectification of an emergency situation are the responsibility of the contractor
RPO, since they are likely to be the only personnel on site with an intimate knowledge
of the technical procedures.

10.2 Source lost or dropped out of exposure device.

The area where the source is suspected to be located shall be cordoned off at the 2.5
µSv/hour radiation level. When the area has been secured the radiographer shall
contact the R.P.O. for further advice and instructions. Survey meters shall be used to
conduct a thorough survey of the area to locate the source. The barriers to the
radiation area should be adjusted as necessary after the source has been located.

The source shall be retrieved with handing tongs and placed in the emergency
container. When all has been secured a full report by the competent person and the
radiographer concerned shall be forwarded to the head office within 24 hours.

10.3 Leaking or damaged source

The radiographer shall immediately clear the area of all personnel. The area shall be
cordoned off and guarded at the 2.5 µSv/hour radiation level. After the area has been
secured the competent person shall be notified and he will give further advice and
instructions as necessary.
If the source is outside of the exposure device then it shall be retrieved and placed
in the emergency container using handing tongs. When all has been secured a full
report by the RPO and radiographer shall be forwarded to head office within 24 hours.

10.4 Emergency condition involving X-Ray Equipment

SHUTDOWN OF X-RAY

X-RAY WITH THE USE OF EXTERNAL POWER SUPPLY

During X-ray activities with the use of external power supply, the following steps
should be taken when X-ray equipment will not turn off according to the setting:
 Make a barrier at a distance where the dose rate is not higher than 7,5 µSv/h by
measuring it by the survey meter;
 Make sure everybody has left the area within the restricted area;
 Wait until the battery is discharged;

X-ray with electrical supply
During X-ray activities with electrical supply, the following steps should be taken when X-
ray equipment will not turn off according to the setting:
 Remove power supply by pulling out the plug and equipment will automatically
shutdown;

 Report incident according HSE protocol of the DE HE vessel

10.5 ISOTOPE DROPPED OVERBOARD

During routine NDT for tie in at the rear of the vessel on board a vessel the isotope is dropped
overboard. Or is dropped when handling between work sites.

Key Symptoms:
 Source container missing lost or witnessed falling overboard

Key Causes:
 Operator error
 Mechanical impact.

Typically when tie in welds are done the vessel will remain stationary. Isotope storage areas
will ensure source will not be movable during adverse weather / transit conditions.
If the isotope has been dropped into the sea there will likely be no exposure risk to personnel.
Radiographic sources are sealed so contamination leakage risk is low however containers may
contain depleted Uranium.

Action to be taken by the radiographer and RPO

The worker must take the following actions immediately:

k) Ensure no exposure to personnel is possible if applicable and make safe.
l) Alert the vessel immediately.
m) Arrange for relocation of the dropped container via the vessel using a suitable means such
as a Buoy or GPS co-ordinates.
n) Write an interim report detailing circumstances and substance specific details such as
activity / equipment numbers etc.
o) Report the incident to the Company Senior RPO.

Action by Senior RPS/ Employer

p) Investigate and alert regulators as required.
q) Liaise with regulators to recover the isotope and container or resolve the incident.

When doing radiographic work, a calibrated functioning radiation monitor must be

used at all times to continuously monitor all actions with radiation sources;
Daily radiation levels must be measured and reported as per according to the IRR17;
When radiographic work is carried out in places not specifically equipped for that
purpose, the radiation area must be cordoned off with tape and warning flags. During
radiation, the demarcation must be continuously supervised and the dose rate at the
demarcation checked;
On the demarcation line the dose rate shall be as low as reasonable achievable
(ALARA) but may not exceed 7,5 µSv/h for the controlled area and 2,5 µSv/h for the
watched area;
If, in spite of all precautions, a person enters - or there is a threat of someone
entering - the demarcated radiation area, the radiation activities shall be stopped
immediately;
When working with radiation sources, the correct position of the source must
regularly be checked by a radiation monitor
With an X-ray tube make sure the tube no longer emits radiation after having been
switched in neutral position;
A radiography crew, each shift, consists of a minimum of 2 people. Only if there is
a physically impenetrable demarcation in place, a crew can consist of only 1
person, e.g. in a bunker.
At least one of the crew members must have Radiation Protection Safety
certificate;
At least one of the crew members must have RT level 2 according ISO 9712/EN
473;
Both must be in possession of a radiation monitor. An exception to this rule applies
to working with X-ray equipment of low energy <50KeV. In general, the
demarcation area will be so small that a second radiation monitor (ionization
chamber) is not required;
The transport of radioactive sources is only permitted in specially equipped
vehicles and must always be accompanied by the required transport
documentation;
All safety aids available are to be used when doing radiographic work. Also, this
procedure must always be at hand in the work area. Each radiographer and helper
must be in possession of this procedure;
Radioactive sources and X-ray equipment shall never be left unattended. After
work it has to be locked as per procedure (e.g. storage bunker, storage pit).

Each radiation incident shall be reported by the person directly involved. If his
immediate supervisor cannot immediately be contacted, he himself shall report it
to the HSE Officer, Vessel Management.
As Low As Reasonable Achievable
Exposure of any part of the human body to X-rays or gamma-rays can be highly
injurious to health. Wherever X-ray equipment or radioactive sources are in use,
which can emit ionizing radiation with a operating potential difference higher than
5kV, appropriate legal requirements shall be applied
Established safety precautions shall be strictly observed by everyone working with
ionizing radiation.
For radiation monitoring only calibrated dose rate meters shall be used. Maximum
radiation at the border, marked with radiation warning signs, of the radiation
working area shall be in accordance with local regulations and detailed into the
specific procedure. Safety equipment shall include:
 Film badge,
 Survey meter,
 PED Personal Electronic Doses-meter
 Flashing & audible alarms,
 Barricades
 Radiation barrier/tape Black& Yellow
 Radiation on warning signs “supervisor signs” includes the RPS contact details.
 Controlled area radiation warning flags, including the maximum dose rate at the
barrier.

maximum allowed level, the exposure shall be stopped immediately and the
demarcation of the controlled area shall be expanded and/or the radiation source
repositioned in such a way that maximum levels are not exceeded.
Access to this area is restricted to classified personnel whilst exposures are taking
place. A physical barrier (e.g. tape) and warning signs shall indicate the perimeter
of the area.
The area must extend over as small an area as practicable and not extend to such
a size that it cannot be effectively supervised/patrolled. Ideally the radiographer
making the exposure should be able to see the whole of the controlled area from
the switch board for X-ray or wind-out position for Gamma-source.
Yellow/black tape or other site agreed tape/ribbon/strip shall clearly define the
controlled area. The physical barrier shall have sufficient warning signs in close
proximity that exhibit the radiation trefoil, the words ‘Controlled Area’, the hazard
(radiation) in English and give informative information in English (i.e. do not pass
barrier).
Natural boundaries may be utilised as the controlled area periphery although
consideration must be given to patrol and monitor such boundaries to prohibit
intrusion.
In determining the size and shape of the controlled area, the effects of scattered
radiation shall be taken into consideration. One of the effects of scattered radiation
is that higher dose rate reading may be apparent as you move away from the
source. Special attention shall be paid to beam direction to ensure that the
controlled area shall be as small as possible to patrol.
In case the scatter radiation is towards the previous or next station additional
shielding must be taken in to account. In no circumstance non Radiation workers
are allowed to receive more than 2,5 µSv/h.
Watched Area or supervised area (outside the DOGHOUSE to the tape barriers)
The watched area is that area where the access is prohibited for any staff other
than the radiography crew. For the watched area the dose radiation may not
exceed 2,5 µSv/h.
Uncontrolled area
For non classified / non radiation workers
For the scope of work an estimated of 24 welds are calculated per shift of 12 hours.
The time for radiation will be set at no more than 2.5 minutes per weld. The barrier
will be set at a maximum of 2.5 µSv/h. A non radiation worker can receive a
maximum of: 2.5 µSv per 24 hours
