1. SCOPE
This procedure establishes the rules and guidelines for the safe use and handling of
gamma radio-isotope including both Ir-192 and Se-75 radio-active source and X-ray
(External and Internal) in order to prevent hazardous conditions for both users of
radioisotopes and the general public. Also, describing the appropriate actions to be taken
during radiation emergency situation.
This procedure also specifies the requirements to minimize the risk of exposure from
industrial radiography while transporting, handling, storing and using projectors with radio-
isotope.
Personnel shall pay due regard to the safety instructions and procedures set out in the
contents.
This procedure shall be used in conjunction with the Company General Local Rules &
Contingency Plans for Site Radiography.
If at any time any employee is uncertain of his responsibility he should not hesitate to
contact a member of management.
This procedure is intended to furnish industrial radiography for both onshore and off-shore
QP premises or controlled contractor’s premises
POLICY
Radioactive source and devices (both gamma camera and X-ray tubes) shall be handled,
transported and used in a safe manner to minimise the risk of radiation exposure to client’s
personnel, contractors/ subcontractors personnel and the general public.
ICES shall not use radioactive source strength greater than maximum permissible limit as
per project specific requirement.
This procedure is based by the Law by Decree No. 31 of the Year 2002 On

Contract Holder : A person appointed within the Sponsor department who is responsible
for making and managing all aspects of the contract. Often referred to
as the job officer.
Conveyance : Vessel or any hols, compertment, or defined deck area of a vessel for
transport by water
Exposure : Means the amount of ionizing in air from x-ray and gamma rays
Limits : Radiation source limits used for the purpose of radiation protection,
specified by Ministry of Municipality & Environment
Offshore : QP-Offshore
Radiography work Permit: A document issued for the performance of work involving radioactive
source
Sender : Any person or department who prepares a consignment for transport and is
named in the consignment documents
Package Type A: A package which provides a safe, economical means to transport a relatively
small but significant quantity of radioactive material. It shall be able to endure
the normal conditions of transport such as drop from a vehicle, exposure to rain,
impact by a sharp tool, and stacking
Package Type B: A package which is able to endure most of the outside factors without its
radioactive contents being leaked of radiation to the general public or rescue
personnel
Absorbed dose: A unit of measure for the amount of energy deposited by ionizing radiation in a
material per unit of mass.
Activity: A unit of measure for the number of radioactive disintegrations per unit of time.
ALARA: As Low As Reasonably Achievable. This term is used specifically for radiation risk
reduction by the International Atomic Energy Agency (IAEA) to make reasonable efforts
to maintain exposures to radiation to the lowest level possible as is practical consistent
with the purpose for which the activity is undertaken.

It is prohibited to use a radioactive source with strength greater than 25 Ci for QP
Operations (Offshore and Onshore) without prior approval from MME.
Radiation Worker
When required, QP shall employ suitably qualified contract personnel as Authorised Persons to
carry out the Industrial Radiography. These Authorised Persons are normally Radiography
Technicians or Specialists who use the Radiography Technique to produce radiography films,
which aid inspection related interpretations. QP shall accept those trained and approved to
recognised standards or certification. They shall be capable of demonstrating their ability to
Becquerel (Bq): The international system of radioactivity, equal to one nuclear decay or other
nuclear transportation per second.
Collimator: A small radiation shield of lead or other heavy metal used in radiography. A collimator
placed on the end of the guide tube has a small opening through which a narrow cone of
radiation escapes when the radioactive source is cranked into the collimator. Use of a
collimator can greatly reduce the size of the controlled area to which access must be
restricted.
Collimator Area: A Controlled Area is one where the exposure of an employee aged 18 or over is
likely to exceed threetenths of the annual dose limit (20 mSv/year).
Dose Equivalent: A measure of the biological impact from ionizing radiation to humans.
Defined as equal to the Quality Factor (QF) times the Absorbed Dose.
Dose Equivalent Units : Sievert (Sv)= Quality Factor (QF)XAbsorbed Dose (Gy).
rem = Quality Factor (QF) X Absorbed Dose (rad).
1 Sv = 100 rem
1 rem = 0.01 Sv
Go/No Go gauge - Refers to an inspection tool used to check a work piece against its allowed
tolerances.
Ionising Radiation - Charged particles (alphas, betas, positrons and protons), neutrons, gamma
rays and x-rays capable of creating damage to human cells through the
ionization of chemicals within the cell.
A suitably trained and experienced person, qualified in Radiological Work and appointed in writing
by an authorised licensee, to be a Radiation Protection Officer. The person shall hold a valid
personal licence issued by the MME
Supervised Areas
A licensee shall designate the supervised areas. These are the areas in which the occupational
exposure conditions need to remain under review, even though no certain measures for protection
and safety are normally required.
4.1. General
It is the responsibility of all personnel to comply the companies safety policy at all
times and to follow the CLIENT safety and permit requirements when on site and at
any other location where work for the Project is conducted.
All radiation workers shall hold MME license and comply with client’s specific
requirement in regad to radiation safe work and .
The role of the RPE is to advise on all matter relating to radiological protection and
legislation and to give advice in the case of an incident involving radiation.
Any employee who because of the nature of his work is likely to be exposed to ionizing
radiation shall be designated as a classified worker.
All classified personnel shall wear both a TLD and a pocket dosimeter.
A person who must has knowledge, experience and capable to deal with any
emergency situations involving radioactive materials. The RPO shall have a
qualifications, training in order to oversee the implementation of the prescribed
radiation protection regulations and give advice on radiation safety and protection.
The individual must have at least university degree in engineering/science or diploma
(two years at least after high school) in a discipline of engineering or science record
and should be certified and qualified by MME and shall be appointed in writing as
RPO

ICES nominated RPO shall be responsible planning and supervising all the radiation
work activities, identifying the associated hazards and assessment of risk to involved
persons and member of public, implementing necessary control measures to mitigate
the risk, and providing specialist advice on the subject of radiation protection.
ICES RPO shall aso be well conversant with international regulations on radiation
protection and shall have knowledge of Local Radiation Protection Regulations as
well as the requirements laid down in QP standards.
Planning of all work activities involving the use, handling, storage and transportation
of radioactive material and Radiation Equipment.
Conducting risk assessment of all radiation work activities to identify all the
associated hazards and recommend appropriate control measures.
To ensure that all the recommended control measures are effectively implemented.
To supervise and control all the aspects of work involving receipt, storage, use,
handling and transportation of radioactive material and radiation equipment.
To identify and classify the controlled and supervised areas for the radiation work
site and ensure adequate physical barricading with appropriate warning and/or
safety signs.
To ensure that only authorized, trained and qualified Radiation Workers are
deployed for the radiation work.
To ensure that all the radiation workers are provided with personal dosimeter. All
the records of individual dosimeter results shall be maintained up to date.
To prepare job specific operational procedures for radiation work activity
incorporating adequate protective precautions.
To ensure that all the personnel involved in radiation work received adequate
training commensurate with the nature of their job and they are familiar with all
instructions and decisions with respect to radiation protection.
To suggest, design and implement methods for the mitigation of radiation risk for
workers, member of public and adjacent facilities.
To ensure that all the concerned parties, including the asset owner of adjacent
facility, been informed of the radiation work activity, necessary approval and/or
concurrence obtained to carry out the work. \
To develop and supervise the implementation of following radiation protection
programs and maintains up to date records.
- Personal Dose Measurement
- Disposal of Radiation Wastes.
- Radioactive Material Storage Instructions
- Radioactive contamination and leakage monitoring.
- Radiation Protection Training and Awareness Program
To identify and meet the radiation protection requirements of protective
equipments, radiation monitoring devices, and audio/visual warning signs.

To ensure the full compliance with the QP Standard and all applicable regulations.
The RPO shall be responsible for the supervision of all personnel involved in the use
of radioactive materials and the enforcement of all instructions and safety standards.
He shall make frequent checks at the beginning of each working day on the zeroing
and recharging of dosimeters and on the condition of all equipment. He shall make
field audits to ensure that all instruction and standards are compiled with.
The RPO is also responsible for the emergency procedures whenever there is a
malfunction of equipment involving a potential radiation hazard. In the event of an
accident he shall assume control as soon as possible ensuring that all necessary
precautions have been put in motion, statements taken, and a radiation report
completed.
The RPO shall obtain film badges from the area office for distribution to the
radiographers, and arrange for the prompt return of used badges to the Office.
The RPO shall send a copy of all sealed sources information to the Office. He shall
issue identity cards to the classified workers. He shall ensure that one calibrated
radiation monitor is always on standby at the base and maintain the emergency kit in
good order at all times.

Radiographers are classified personnel who are directly concerned with ionizing
radiation in their work and are certified to carry out the radiography. Radiographers
must be properly trained on radiation safety and have at least high school secondary
certificate as their basic educational qualification. All radiographers shall carry valid
MME license for handling radio-isotopes
They shall ensure that all boundary marks, sign, audible warnings and warning lights
are in place and operating in accordance with the regulations, and that the area is
clear before exposing the source.
At the beginning of each working day the radiographers shall ensure that all
equipment is in a safe working condition. All malfunctions shall be reported to the
supervisor or RPO immediately.
Radiographers shall wear a valid film badge and a pocket dosimeter that has been
zeroed and recharged at all times when working in a radiation area. They shall ensure
that the isotopes are always locked and stored safely upon the completion of the day’s
operation and are returned to the correct storage bunker.
They should always monitor the guide tube and projector before and after operating
the projector.
They shall notify immediately the RPO of accidents or incidents, including suspected
over exposure, or a leaking isotope.
Non-classified persons are members of the general public who are not permitted to
be involved in work within or passing the barrier where ionizing radiations are present.
Non-classified persons shall not be allowed to enter a potentially hazardous radiation
zone (in excess of 2.5 µSv /hr).
Before any Radiographer can carry out any radiographic duty, they shall be authorized
to do so in writing by the Permit Controller, and Safety Officer or Site Representative.
Further, a risk assessment shall be carried out before involving any ionizing radiation
work.

General risk assessment and Task Risk Assessment should be consulted (see
HSE-plan) during assessment. If these assessments are considered applicable,
taking into account any site specific risks, and acceptable then it shall be adopted.
When a risk assessment identifies that existing preventive or protective measures
are inadequate, additional actions to eliminate/reduce risks shall be implemented
(generally additional/local shielding, collimation or beam direction) and a project
specific risk assessment shall be produced.
5.2.1 Job Safety Analysis (JSA)
Before commencing any RTR / Radiographic work, the JSA must be read and in
place. This must be followed up by a toolbox talk. The recorded toolbox is
mandatory to obtain the permit to work.
5.2.1.1 Permit to work
If CLIENT’s / CONTRACTOR’s procedure or safety system defines a permit to
work is required the radiography team shall follow the system established at site.
The RPO radiographer responsible for the work location shall sign for acceptance
of the conditions within the work permit and make sure these acceptances are
obeyed and known by the crew.
The RPO should ensure that no requirement of the permit contradicts the
requirements of the local rules or any other mandatory requirement. If in doubt, he
should consult with the client’s representative at site.
6 PERSONAL MONITORING
Approved film badges / T.L.D. shall be worn by all classified workers engaged on work
involving the use of ionizing radiation. Film badges / T.L.D. shall be issued on a two
months basis. At the end of the period, film badges / T.L.D. shall be returned to the
head office together with the personal dosimeter records. The film badge results shall
be entered on the personal dose record and kept in the personal files. Film badges or
T.L.D shall be sent to MME for evaluation.
The badges shall be worn as follows: - place the badge in the holder with the serial
number clearly visible throughout the front window of the holder. Pin the holder to the
outer clothing at waist level at the front of the body, with the back of the holder nearest
the body. The film/TLD badge must be worn at all times during working hours.
Personal Dosimeters shall be worn at all times during working hours by all workers
engaged on work involving the use of ionizing radiation. Personal dosimeters shall
have a measuring range of 0-2000 µSv. Personal dosimeters shall be zeroed and
recharged at the beginning of each working day. A personal dosimeter rate record
shall be maintained by individual radiation worker on completed each day worn.
Radiation Survey Meter is the term used to describe an instrument capable of
measuring radiation dose -rates or surface contamination. Survey Meter is scaled in
units of dose-rate, µSv/h. This allows the measurement result to be read directly from
the display. It should be properly maintained, tested and calibrated every year with
reference to standards traceable to national or international standards.
Each radiographic crew shall have at least two radiation survey meters and use it
during all stages of radiographic activities.
Radiation Survey meters used for monitoring shall be in good working condition and
be able to measure from 2μsv/hr (0.2mR/hr) to 10mSv/hr (1000mR/hr).
Calibrated survey meters shall be stickered clearly indicating the make/model, serial
number, calibration date and expiry date. Calibration records shall be maintained at
office. Intermediate calibration shall be carried out whenever repairs and/or
adjustments are made due to handling.
In case of the survey meter becoming inoperable condition, the area shall be
barricaded off and all the radiographic operations shall be suspended until
replacements are available.
Radiographer shall check the survey meter on daily basis before start of work for
on/off switch, battery check, working condition and allow to warm up.
(1) For persons working in the classified area, the accumulated whole body
dose shall not exceed 20 mSv / per year .
(2) For non classified persons, the accumulated whole body dose: 1mSv/Per year
(3) A person, who has received a dose of radiation in excess of the limit Mentioned
in paragraph 6.3 (1) above, shall not be allowed to be involved in radiation work
for a period of 12 months after the dose received exceeds the limit. Further
investigation as well as necessary medical examination is to be carried under
the guidance of radiation expart in order to set recovery action.

6.4 Records
(a) Daily records: A daily record of all personal dosimeter readings shall be kept by
the radiographers. The RPO shall ensure that these records are maintained and
kept up to date. The records shall be forwarded to Head Office every month
and kept in the radiographer’s personal file (See Appendices). Any daily
exposure indicated by the dosimeter to be above the allowable limit (over 100
mR) shall be reported immediately to the RPO. The RPO will conduct
investigation and make a written report. Medical examination will follow to know
appropriate action to be taken.
(b) Records: Records of film/TLD badge readings shall be kept at Head office.

Wipe test shall be carried every two years by an MME approved party .
Engineering controls involves the use of shielding, warning alarms, warning lights,
barricades etc.
7.3.1 Time
7.3.2 Distance
The radiation intensity from a source inversely proportional to the square of the
distance from the source. This is known as the inverse square law and can be
expressed generally as:-
Shielding can greatly reduce radiation exposure and any available shielding should
be used to advantage. Collimators should be used whenever possible.
It may for instance be possible to work behind a concrete structure which would
provide shielding from the radiation. In field application many items are available for
use as shielding. A useful concept for estimating shielding efficiency is the half / tenth
value thickness which is defined as the thickness of shielding material required to
reduce the radiation to one half / tenth of its original value. A table of half /tenth value
thickness for common shielding material is shown in Appendix B
MORE SHIELDING = LESS EXPOSURE
The calculations are intended for guidance only. All situations involving exposure of
ionising radiation must be monitored with survey meter to ensure that personnel and
barriers are at the required distance from the source. USE ONLY ONE SYSTEM OF
UNITS THROUGHOUT CALCULATION.
Above distance in column II will be further reduced, as the beam has to penetrate through
the walls of the pipe being radiographed, this reduction will depend on the wall thickness.
The above distances are calculated for no shielding other than the tungsten collimator
used. Actual site conditions will reduce these distances i.e. Pipe work, columns, structural
items, walls, etc., in line with item being radiographed.
Every radiographer shall wear a dosimeter and valid TLD badge. Two properly
calibrated and operational radiation survey meters shall be available for each
source/crew to be used. The radiographic exposure device shall be surveyed on all
sides to ensure that the source is in the correct storage position, this will also verify
that the survey meter is functioning correctly and provide a baseline reading for
comparison with future measurements. The radiation intensity should be no more
than the following:-
At 5 centimetres from the surface
The average dose rate shall not exceed 50 µSv /hour
The maximum dose rate shall not exceed 100 µSv /hour
At 1 meter from the surface
The average dose rate shall not exceed 2 µSv /hour
The maximum dose rate shall not exceed 10 µSv /hour

The radiographer shall check for obvious defects and damage to the exposure device,
control unit and guide tube(s).

8.2 Transportation
The radiographic exposure device shall be transported to the work site with the safety
locks in position. It shall not be transported in the cab of the vehicle but in a storage
box affixed to the vehicle. The box shall have warning in English & Arabic. The vehicle
shall be driven by classified persons with calibrated survey meter. The maximum
reading shall 2 mR/hr at surface of storage box. The radiation warnings signs shall
be on the box. And the vehicle shall be clearly marked with the radiation warning
placards (3Nos.), one each side and one on back of vehicle. The vehicle shall be also
marked with emergency contact information providing contact details in the event of
an accident.
Under no circumstances is the equipment to be transported in the assembled or open
position. Only Classified persons are permitted to travel in the vehicle. Person
transporting radioactive material should keep copy of use exclusively licence issued
by the Authority (MME). Permit for transport of the radioactive materials issued by
Local authorities.
Vehicle Registration certificate.
Driving Licence, A copy of driver’s passport.
Driver shall ensure the vehicle shall be equipped with following equipment
The Company uses the following equipment for industrial radiography and
each equipment has a unique number to facilitate record keeping in respect
of movement, location and for maintenance purposes.
Sealed sources
Proper demarcation with flags, tapes and rotating or flashing lights must be in
place. On the demarcation line the dose rate may not exceed 7,5 µSv/h;
Reduce the beam width (e.g. defragmentation) as much as practically possible;
Work as quickly (duration) and safe as possible;
Demarcations must be partially removed every time radiation workers are leaving
the work area (mainly open and close each weld) and taking the key of the switch
board which shall be kept only by the employer’s appointed RPO.
Check all equipment each day for proper function before start of work, and if
applicable, apply maintenance according to maintenance procedure.
Operating Procedure - Wind out Type Container (Gamma Ray Equipment) and
X-Ray equipment
(b) Position the source stop at the appropriate radiographic focal point. A collimator
should be used whenever possible, with the source stop to reduce the size of
the restricted area and to minimise operator exposure.
(c) Lay out the guide tube(s) as straight as possible, never attempt to use more
than guide tube section. The bend radius of the guide tubes should not be less
than 50 centimetres

(d) At the end of the guide tube position the exposure device. The location of the
exposure device and the path of the guide tube should be chosen to afford as
much shielding as possible to the operator.
(e) From the exposure device lay out the wind - out unit so that the operator is
behind as much shielding as possible. The bend radius of the guide tubes
should not be less than 92 centimetres.
(f) Determine the barrier distance using the formula given in Para 7.2 from the
source stop to the boundary of the high radiation area (7.5) µSv /hour). Rope
off this area and post the boundary with warning signs. Verify the area with
survey meter on first exposure

(g) At the exposure device connect the driving cable to the source holder assembly
and connect the control unit to the exposure device. Leave the exposure device
in the LOCK position. Connect the guide tube to the exposure device. Layout
drive cable and front tube to be check for obvious damage to the equipment.
(h) Check to ensure that there are no unauthorised personnel inside the restricted

area. Thoroughly check all cable connections, bend radii and source stop
location. Unlock the exposure device and set it to the operation position.
(i) Place the Survey Meter by the Control Unit. Survey Meter should be properly
maintained, tested and calibrated every one year with reference to standards
traceable to national or international standards.
(j) At the control, observing the SURVEY METER, rapidly rotate the crank in the
expose direction. When the source emerges from the exposure device, there
should be a rapid increase in radiation intensity. The intensity should gradually
decreases as the source moves towards the source stop. As the source enters
the collimator, there should be a significant drop in radiation intensity. While
the source is in the exposed position retreat to the boundary of the restricted
area.
(k) Survey the boundary of the restricted area to ascertain that it was correctly
established. Adjust the barrier ropes as necessary to maintain safe radiation
level of 7.5 µSv /hour at the boundary.
(l) During the exposure time maintain a constant surveillance of the restricted area
to ensure that no unauthorized personnel enter the area.
(m) At the conclusion of the exposure time, approach the control unit WITH THE
SURVEY METER, WHILE OBSERVING THE SURVEY METER, rapidly rotate

gradually increase as the source moves towards the exposure device. As the
source enters the exposure device, the intensity should rapidly decrease to
background levels.
Survey the device on all sides paying particular attention to the exit port area.
The radiation intensities should be approximately the same as the initial survey.
The radiation intensity should rapidly decrease as the survey meter is moved
away from the exposure device.
1. The three man team will arrive at the site with appropriate Permit.
2. Team will organise their equipment including the barriers, signs, lights and X-
ray tube and Crawler.
3. The team will evacuate the area and erect barriers and signs at the pre-
calculated safe distance based on our experience i.e. 25- 300 meter for 300 Kv
depending on the focus direction and shielding . If required, additional
personnel may be requested from the Client to police the supervised area.
4. The working place will be prepared in accordance with the Manufactures
Operating Procedure, Radiographer shall barricade the controlled area
ensuring that the controlled area is secure, and that no unauthorised
personnel will enter.
5. After all the setup of control panel and X-ray tube. Radiographer will start the
exposure and will check the radiation level at the barricade that should be 7.5
µSv/hour. If the radiation level found higher than the 7.5 µSv/hour then
barricade distances will be increased until the safe level.
7. After completion of the Exposure, the equipment shall be switched off and
removed from site. Radiographs will be taken out and will be transferred to the
processing area/dark room Located at site.
To prevent access to any radiation area, warning signs, barriers, flashing lights are
to be posted to warn member of public about the Radiation Hazard

(a) Follow the manufactures recommended practice for opening the container of
the isotopes.
(b) Attach the guide tube to an empty tube in the source container. Lay out the
Guide tube as straight as possible.
(d) From the projector lay out the control unit so that the operator is behind as
much shielding as possible.
(e) At the projector, connect the driving cable to the source holder assembly and
connect the control unit to the projector.
Leave the projector in the LOCK position.
Connect the guide tube to the projector.
(f) Source change shall take place at ICES Isotope storage Pit.
(g) Check to ensure that there are no unauthorised personnel inside the restricted
area. Thoroughly check all cables connections. Unlock the projector and set it
to the OPERATE position.
(i) At the control, observing the survey meter, rapidly rotate the crank in the Expose
direction. As the source emerges from the projector there should be a rapid
increase in radiation intensity. The intensity should gradually decrease as the
source moves towards the container. When the source enters the radiation
intensity should fall to background levels.
(j) Approach the container with a survey meter. If all is clear, uncouple the guide
tube, and then disconnect the source from the driving cable.
(k) Connect the new source to the driving cable; connect the guide tube to the
source container.
(l) Check to ensure that there are no unauthorised personnel inside the restricted
area. Thoroughly check all cable connections.
(m) At the control, observing the survey meter, rapidly rotate the crank in the retract
direction. As the source leaves the container there should be a significant
increase in radiation intensity. The intensity should gradually increase as the
source moves towards the projector. As the source enters the projector, the
intensity should rapidly decrease to background levels.
(n) Approach the projector with the survey meter. Survey the projector on all sides
paying particular attention to the exit port area. Survey along the guide tube to
the source container. When it has been determined that all is safe LOCK THE
PROJECTOR.
(o) Disassemble the guide tube and replace the cover to the container following the
manufacturer’s instructions. Monitor the container after the cover has been
replaced.

10 EMERGENCY PROCEDURES
10.1 Introduction
Emergency conditions are those in which the source cannot be returned to the
shielded position by normal means. The most important factor in an emergency is to
recognize that an emergency exists. This can only be accomplished by the proper use
of a survey meter which can give valuable information as to the nature of the
emergency, (e.g. whether the source is jammed in the collimator, guide tube or exit
port). Each emergency should be treated individually and the entire operation thought
through from beginning to end before any action other than erecting barriers and
informing the correct authorities, is taken. Always use as much shielding as possible
without hampering the emergency operation. In accomplishing emergency
procedures it may be necessary to work in high radiation intensities. Determine the
amount of time necessary to be in these areas and compute the expected exposure
before performing the operation. Only thorough careful planning and application of the
principles of time, distance and shielding can emergencies be dealt with safely.
Within the next paragraphs the instruction are given on how to shutdown in case of
emergencies. If none of this instruction apply and/ or give the desire effect, please
contact the emergency response phone as it must be written on the supervisor

During an emergency situation the following items shall be available and provided By
contractor as per reference (Safety Reports Serial No.13 IAEA 1999):

Self-reading dosimeters for each team member.

Source
Emergency action is the overall responsibility of the MME. However, the initial steps
for the rectification of an emergency situation are the responsibility of the contractor
RPO, since they are likely to be the only personnel on site with an intimate knowledge
of the technical procedures.
The area where the source is suspected to be located shall be cordoned off at the 2.5
µSv/hour radiation level. When the area has been secured the radiographer shall
contact the R.P.O. for further advice and instructions. Survey meters shall be used to
conduct a thorough survey of the area to locate the source. The barriers to the
radiation area should be adjusted as necessary after the source has been located.
The source shall be retrieved with handing tongs and placed in the emergency
container. When all has been secured a full report by the competent person and the
radiographer concerned shall be forwarded to the head office within 24 hours.
The radiographer shall immediately clear the area of all personnel. The area shall be
cordoned off and guarded at the 2.5 µSv/hour radiation level. After the area has been
secured the competent person shall be notified and he will give further advice and
instructions as necessary.
If the source is outside of the exposure device then it shall be retrieved and placed
in the emergency container using handing tongs. When all has been secured a full
report by the RPO and radiographer shall be forwarded to head office within 24 hours.
SHUTDOWN OF X-RAY
During X-ray activities with the use of external power supply, the following steps
should be taken when X-ray equipment will not turn off according to the setting:
Make a barrier at a distance where the dose rate is not higher than 7,5 µSv/h by
measuring it by the survey meter;
Make sure everybody has left the area within the restricted area;
Wait until the battery is discharged;

During routine NDT for tie in at the rear of the vessel on board a vessel the isotope is dropped
overboard. Or is dropped when handling between work sites.
Key Symptoms:
Source container missing lost or witnessed falling overboard
Key Causes:
Operator error
Mechanical impact.
Typically when tie in welds are done the vessel will remain stationary. Isotope storage areas
will ensure source will not be movable during adverse weather / transit conditions.
If the isotope has been dropped into the sea there will likely be no exposure risk to personnel.
Radiographic sources are sealed so contamination leakage risk is low however containers may
contain depleted Uranium.
Refer ICES Doc ICES-HSE-06 Rev 0 for the Radiation Emergency Plan in detail.
The safe retrieval of radiation source shall be considered as the end of state of emergency. After
the the is returned back to camera or guide tube, the situation shall be informed to HSE authority
at site. The cordon shall be removed along with danger boards and flickering lights.
11 INCEDENT REPORTS
A full report of any incident shall be made and issued by the QP Regional RPO in accordance
with QP Incident Reporting, Investigation manual and MME requirements, the time-scales for
notification depending on the severity of the incident.
12 STORAGE OF ISOTOPES
Radioactive materials shall be stored in approved isotope storage facilities when not in use.
All designated storage facilities shall be approved and licensed by MME before they are
deemed usable. Source of radiation & devices must be properly tracked and recorded by
the RPO. A record shall be kept of the movement of all isotopes into and out of the storage
facility. The storage facility shall be kept locked and secured at all times to prevent any
unauthorized personnel entry except when the radioactive materials are being placed in or
removed from the storage facility.
ICES shall be responsible for acquiring the MME approval for such facility. Storage shall be
in an isolated secured place. The area surrounding the temporary storage place in which
the radiation level exceeds 7.5 µSv/hour shall be restricted by ropes hung with appropriate
warning signs.
13.1 General
The driver shall, as soon as it is reasonably practicable notify the company if he finds
or fears loss of material or is involved in an accident.
The company RPO upon notification of the accident shall inform the police and
MME.
The company RPO shall immediately barrier off the area at the 7.5 µSv/hr distance,
with appropriate warning signs and lights.
Assess and monitor the area for possible damage to the container and ascertain if
the source is exposed.
If all is clear, retrieve the exposure container or transport container from the
vehicle. Move it to a safe location for investigation.
All personnel involved in the accident shall be sent for adequate medical
surveillance.
The source projector shall be returned to the manufacturer or qualified expert for
detailed testing and servicing.

(a) A placard (Appendix F) shall be displayed on each side and the rear of the
vehicle. The placard shall be removed immediately when the vehicle is no
longer transporting radioactive material. A calibrated survey meter shall always
accompany the radioactive material.
(b) A fireproof notice shall be displayed where it is plainly visible to the driver, but
not so that it obscures his vision. It is only to be exhibited when the vehicle is
carrying radioactive material. (See Appendix G)
13.4 Driver’s responsibilities
The driver of a vehicle carrying radioactive material shall exercise care to ensure that
the material causes no injury to the health of any one and that none of the material
escapes, is lost , or is unlawfully removed from the vehicle.
The driver shall ensure that the packages are not roughly treated and are properly
stored and that the labels and vehicle placard are not defaced.
The driver shall ensure that radioactive material is not stored with prohibited articles
(explosive, flammable materials, corrosives, oxidizing agents, liquefied gases, etc.)
The driver shall not carry any unauthorised and unclassified person in the vehicle.
The driver shall not leave the vehicle unattended out of sight, in any public place
without reasonable cause. The loaded equipment compartment shall always be
locked.
The vehicle shall not be parked for more than one hour, unless there is a clear
space of at least 2 meters all round the vehicle when carrying radioactive
material.
14 TRAINING
All RPO and Radiographers shall receive necessary training through MME
authorized training institute and undergo prescribed examination conducted by
MME for achieving MME licnse for RPO and Radiographer respectively.
ICES shall also organize in-house training program in regular interval for the
development of their radiation workers.The training program shall be organized as

per annual training program. However, unplanned training may also conducted
whenever it is felt deem necessary by Technical Manager.
15 HEALTH SURVEILLANCE
All Personnel employed by the company shall not be considered as classified. Workers who
are directly engaged in activities of radiography are subjected to annual medical
examination and blood examination by an approved laboratory within the preceding 12
months and is certified for in written for such work. The medical examination shall include:
• Complete medical and occupational history including all previous exposures to ionizing
radiation;
• Clinical examination;
An annual medical examination, whose content shall be as above. The examining doctor
shall be provided with the worker’s occupational dose history since his previous medical
examination.
Full Medical records of all classified persons shall be maintained. These records shall be
made available for audit if required by the client / owner.
Whenever the RPO deems necessary, a special medical examination shall be arranged.
Persons not classified i.e. general public, contract personnel, and/or others, are not
permitted to be involved in work or passage where ionizing radiation is present, for this
reason same are excluded from radiation areas where specific barriers and warning signs

ensure that doses to public are below 0.1 mSv / year.
16 EMERGENCY DRILL
In order to combat with radiation emergency situation, ICES shall periodically conduct
emergency drill at radiation work places. An annual Emergency Drill plan shall be
prepared in this regard.
RPO shall prepare a report on the performance of Emergency Drill. The report shall
contain the following as a minimum
a. Emergency situation
b. Equipment Involved
c. Emergency situation handling equipment and accessories
d. Personnel take part in the Drill
e. A brief explanation how the situation is handled with time taken for each action
f. How to monitor the cordon – off location during the time of emergency situation
g. Ending of emergency situation
h. Prepare lesson learned
ICES arranges periodic calibration of its radiation survey meters and personnel dosimeter
by authorized external agencies. The calibration report is reviewed by ICES Technical
Manager prior to implement. A LOG is maintained by Maintenance Engineer for the
calibration status of measuring instrument and equipments.
ICES also maintain a preventive maintenance program for the instrument and equipment.
The breakdown instrument (X-ray Generator, Control Pannel etc.) are send to outside
agencies for repair or servicing when in-house system is unable to take necessary
corrective action.

18 INVENTORY MANAGEMENT
An accurate accounting system is maintained for individual sources. Each source record shall include
the serial number, receipt date, decay chart, test report, projector or transit container used, daily
location.
Daily movement of sealed sources shall be recorded in the store/site log book and shall be signed
in and out by the responsible authorised person
Regulatory authorities and client representatives on projects will be provided with the inventory
details as applicable.
Prior to importing or exporting all radioactive material and associated equipment including X ray
equipment approval from regulatory body is obtained. This is forwarded to the supplier prior to
activating the shipment.
The decayed sources are returned back to radio-isotope supplier for the safe disposal.
Upon receiving a sealed source whether new purchase or transfer from another
location then the following information will be verified and maintained on a daily or job
by job basis to indicate the location and authorised person having responsibility for the
source movement.
a) Date of receipt.
b) Source serial number.
c) Type of nuclide and activity.
d) Transport documents.
e) Assigned projector/container serial number.
f) Location and authorised person’s name and signature.
Radioactive sources that are no longer used or with low activity (below 5 Curie) will be exported
back to supplier for safe disposal. Such sources will be stored in the permanent facility until
administration procedures, Regulatory body approvals and shipping procedures are finalized
for export. The following information will be maintained for all disposals of radioactive materials.
a) Date of dispatch.
b) Destination.
c) Source serial number.
d) Type of nuclide and activity.
e) Method dispatched.
f) Assigned projector/container serial number.
g) All necessary approvals from regulatory body
Investigation level for ICES radiation workers is set as 1.0 mSv / month.

After a joint review of the full Investigation Report, the Radiation Safety Committee shall decide
on any further course of action.
The Operations Manager, who shall periodically review and note any adverse trends, shall
retain these reports. These trends shall be reported to the HSEQ Co-ordinator. Following a joint
review, if required, actions will be decided on to reverse these trends.
Investigation reports will be submitted to MME, and or client representatives on request or as per
regulatory requirements.
If a dosimeter is lost or damaged, or the dose received can't be properly assessed for other
reasons, ADS will be informed. An investigation will be carried out as applicable in line with the
procedure to estimate the dose actually received. This estimate shall be notified to the relevant
authorities along with investigation reports on which the estimate was based and individuals dose
records updated.
If an estimate can't be made a notional dose will be recorded but this will only be used as a last
resort. One or more of the following methods will be used to estimate doses when dosimeters
are lost or damaged.
An official dose is estimated and assigned based on an average from previous monitoring periods
or the highest dose during the individual’s employment.
Dose assigned to the individual based on the dose received by a co-worker performing similar
tasks under same conditions during the monitoring period in question.
Dose assigned after calculating the time spent by individual in radiation area based on known
dose rates. Daily job production details shall be utilized for this purpose
REPORTING PROCESS
The concerned person shall infor the incedent to ICES RPO under his team and also to
operation manager.
The RPO shall investigate the matter and raise a report to operation manager. The report
copy shall be marked to Technical Manager / QHSE Manager.
This Radiation safety and Protection program will be reviewed annually. Audits will be performed
twice a year to verify compliance. Audits will be extended to all ongoing sites as well.
23 SUMMARY
It is compulsory for all RADIATION workers to wear TLD/film badge and Personal
Dosimeter or Personal Electronic Dosimeter when doing radiographic work;

Each radiation incident shall be reported by the person directly involved. If his
immediate supervisor cannot immediately be contacted, he himself shall report it
to the HSE Officer, Vessel Management.
As Low As Reasonable Achievable
Exposure of any part of the human body to X-rays or gamma-rays can be highly
injurious to health. Wherever X-ray equipment or radioactive sources are in use,
which can emit ionizing radiation with a operating potential difference higher than
5kV, appropriate legal requirements shall be applied
Established safety precautions shall be strictly observed by everyone working with
ionizing radiation.
For radiation monitoring only calibrated dose rate meters shall be used. Maximum
radiation at the border, marked with radiation warning signs, of the radiation
working area shall be in accordance with local regulations and detailed into the
specific procedure. Safety equipment shall include:

Controlled Area (DOGHOUSE)
Within the controlled area it is likely that the dose rate exceeds 7,5 µSv/h. If at any
time the radiation level at the demarcations of controlled area exceeds the

maximum allowed level, the exposure shall be stopped immediately and the
demarcation of the controlled area shall be expanded and/or the radiation source
repositioned in such a way that maximum levels are not exceeded.
Access to this area is restricted to classified personnel whilst exposures are taking
place. A physical barrier (e.g. tape) and warning signs shall indicate the perimeter
of the area.
The area must extend over as small an area as practicable and not extend to such
a size that it cannot be effectively supervised/patrolled. Ideally the radiographer
making the exposure should be able to see the whole of the controlled area from
the switch board for X-ray or wind-out position for Gamma-source.
Yellow/black tape or other site agreed tape/ribbon/strip shall clearly define the
controlled area. The physical barrier shall have sufficient warning signs in close
proximity that exhibit the radiation trefoil, the words ‘Controlled Area’, the hazard
(radiation) in English and give informative information in English (i.e. do not pass
barrier).
Natural boundaries may be utilised as the controlled area periphery although
consideration must be given to patrol and monitor such boundaries to prohibit
intrusion.
In determining the size and shape of the controlled area, the effects of scattered
radiation shall be taken into consideration. One of the effects of scattered radiation
is that higher dose rate reading may be apparent as you move away from the
source. Special attention shall be paid to beam direction to ensure that the
controlled area shall be as small as possible to patrol.
In case the scatter radiation is towards the previous or next station additional
shielding must be taken in to account. In no circumstance non Radiation workers
are allowed to receive more than 2,5 µSv/h.
Watched Area or supervised area (outside the DOGHOUSE to the tape barriers)
The watched area is that area where the access is prohibited for any staff other
than the radiography crew. For the watched area the dose radiation may not
exceed 2,5 µSv/h.
Uncontrolled area
For non classified / non radiation workers
For the scope of work an estimated of 24 welds are calculated per shift of 12 hours.
The time for radiation will be set at no more than 2.5 minutes per weld. The barrier
will be set at a maximum of 2.5 µSv/h. A non radiation worker can receive a
maximum of: 2.5 µSv per 24 hours

FIREPROOF NOTICE
1- The metal plate with carved emergency information as shown in below picture fixed in
the driver room and written information about on the carrier company on the external

2. The notice must be securely posted in the cab where it is plainly visible to the driver,
but not so as to obstruct his view of the road, and must not be displayed when
radioactive material is not being carried unless the vehicle is contaminated.
