SUBJECT : INCIDENT REPORT

Sir Randolf,

Roland Malejana, standby technician on duty, at 11:53 pm Roland was informed that there was trouble at
Passenger Elevator no. 6 of OTP 2 and that there were passengers trap inside the elevator. Roland immediately went
to the control panel to check the status of the elevator. Upon arrival at the control panel, he saw error 3601, which was
regularly corrected by resetting the elevator. He tried several times to reset but the elevator was not responding. He
went to look where the elevator car was located. When he saw the position of the car, it was between the first floor
and the second floor, and there was no way he will be able to open the elevator door to rescue the trapped
passengers. He went back to the control panel and tried to use the RECALL and the JEM function buttons but
unfortunately both didn’t function. He tried to reset by removing the batteries and again it did not work.
Around 12:36 am I received a call from Engr Randolf to inform me of the situation. After talking with Engineer Randolf
I immediately called my technician to check the status of the situation, upon learning the situation, I instructed Roland
to try to open the emergency exit. It took them a while to open the emergency exit. When they opened the emergency
exit they saw the ceiling of the elevator. They removed two (2) screws on one side of the ceiling and they pushed the
ceiling down, and put a ladder inside the car and they were able to rescue the trap passengers.

We apologize for the inconvenience this may have caused. In order to prevent this unfortunate event to
happen in the future, I have ordered batteries to replace all the installed batteries. I also have instructed my
technicians to test every two (2) weeks the use of RECALL and JEM functions.
