UNDERTAKING

1. I am, an applicant/permittee of a building permit with the City Building and


Architectures Office (CBAO), City of Baguio with office address at DPS, Utility Road,
Baguio City;

2. I was granted a building permit with Building Permit No. _____________ issued on
__________;

3. The plans and specifications submitted to CBAO which was the basis for such office’s
approval provided for ___ parking slots in consonance with section 707(4) of the
National Building Code of the Philippines (PD 1096);

4. I undertake not to do acts prohibited under PD 1096, to wit;


a. Convert/change the use of occupancy of the parking slots
b. Reduction of parking spaces
c. Encroachment of RROW
d. Public utility and bulky vehicles

5. Should I fail to comply with this Undertaking, Building Permit No. _________ shall
automatically and immediately be revoked or cancelled without any need for without
notice or hearing, in relation to Section 306 of PD 1096 providing for the grounds for the
non-issuance, suspension revocation of building permits;

6. I am executing this undertaking to attest to the truth of all the foregoing, to support my
application for Building Permit / Occupancy Permit and of all legal intents and purposes.
