    Subject: Reimbursement of Emergency medical Expenses

    Dear Sir,

    I am Muhammad Zia ul Haq, Working as Lecturer in IB&M, UET Lahore. On the night
    of Sunday, 3th of September, 2020, I got high temperature and body aches started. Next
    morning, I rushed to the nearest doctor in “Abeer Clinic” situated at Ghoray Shah Road. After
    performing different clinical tests, I was diagnosed with Typhoid fever, high Uric acid and high
    cholesterol. Doctor in the clinic started my treatment and advised me to visit him for five
    consecutive days. Total expenses of this treatment were Rs. 9071. I have attached necessary
    documents and Invoices.

    It is humbly requested to please reimburse these emergency medical expenses bill of Rs.
    9071. I shall be highly grateful to you for this.
