Respected sir,

Sub: - Request for getting transfer to Madina Haram area PHC

I Shyla Ismail Rawther have been working since June 10, 2006 as “Staff Nurse”
in Shugran Health Center, Hanakiya region, under Ministry of Health, Madina
Directorate.

Most respectfully I state my formal request that I need to get transfer to Madina Al
Munawra Haram area health center or MOH hospitals, because my husband can
get transfer to Madina from Dammam, he has been working in Dammam for the
last five years, so it is necessary for me to get transfer to Mainda Haram area,

As I am a lady staff, I am alone in this Shugran PHC, no any other lady nurses
with me, so it is very difficult and fearful to stay alone in Shugran P H C.

Hence I am requesting your kind consideration to facilitate my transfer to Madina

Al Munawra, Haram area.
