I. Program Name, Theme and Location

Name: DIABETES MELLITUS AWARENESS PROGRAM

Theme: “DIYABETIS: ATING ALAMIN AT LUNASAN UPANG MAKAMIT ANG

MASAGANANG PAMUMUHAY” we have to know DM to have a better life

Venue: Bayo, Iguig Barangay Hall

II. Rationale and Objectives

The graduate school students enrolled in Masters of Science of Adult Health Nursing will be
conducting a seminars and/or program activities related to their current course subject: Adult Health
Nursing I. Students responsible for conducting the proposed program are as follows:

4. Jene Christophoer A. Gaspar

As students enrolled in the said course we will be conducting a seminar and program activity which is
entitled as “DIABETES MELLITUS AWARENESS PROGRAM”. The said activity shall enable the participants
to be reinforced with the principles of Diabetes Nursing so that they may maximize these concepts in
their daily practice as Nurses delivering quality nursing service.

The activity shall be an avenue for seeking and achieving the following objectives:

a) To develop a sense of responsibility in organizing an activity/seminar


b) To develop camaraderie among the organizers
c) To imbibe within the participants of being a great instrument and tool in giving therapeutic
holistic care to their patients
d) To exemplify discipline and compassion in their chosen profession and making them more
competitive and competent concomitant to nursing principles.
e) To reinforce the participants’ motives in maximizing their affectivity in giving service to
individuals.

III. Participants
Participants of the said activity will be ______________________________________________

IV. Deliverables and Implementation Schedule

The following activity shall be an activity of the Graduate school specifically to be organized by
the students mentioned above enrolled in MSN as per requirement to their subject course Adult
Health Nursing 3. They themselves shall be the spearhead and project organizers/facilitators.
Below are the guidelines of the activity:

a) The activity shall be implemented on June 18, 2016, the schedule shall be flexible of
implementation depending on any circumstances that may arise which will hinder the activity to
be postponed or moved to another date therein.
b) The group members shall be the organizers and facilitators of the activity.
c) The participants are expected to arrive at the venue 30 minutes before the time. They
are to register to any members of the registration committee.

d) The participants are said to be mandated to come depending on the decision of the Barangay
Captain Emilia T. Bancud, in which the participants shall be informed two days before the
proposed date of the above mentioned training.
e) Time of implementation will be from 8AM – 11:30AM for the audience, speakers and organizers.
f) House Rules shall depend on the facilitators’ prerogative on the implementation style and
sanctions for those who shall not follow.
g) Program of the activity shall be accomplished by the organizers and shall be
given to the audience on the day of the activity.

h) The attendance of the participants shall be checked by the registration committee before and
after the training.
i) The topics to be discussed shall be as follows:

i. ANO NGA BA ANG DIYABETIS?


Ito ay isang pangmatagalang sakit kung
saan tumataas ang blood glucose o asukal sa dugo (hyperglycemia) dahil sa
kakulangan o hindi tamang epekto ng isulin sa katawan.

Ano ang “Glucose”?


Ang glucose ay ang pinaka simpleng sustansya na hango sa pagtunaw ng ating
kinakain; at ito ay ginagamit ng ating katawan upang magkaroon ng lakas.

ANO ANG INSULIN?


Ang Insulin ay isang “hormone” mula sa PANCREAS na nasa loob ng ating tiyan. Ito
parang susi upang makapasok ang asukal mula sa dugo ng tao papunta sa kanyang
mga laman.

ii. MGA URI NG DIYABETIS


Unang klase ng diabetes (Type 1-Insulin Dependent)
 diabetes ng kabataan
 Hindi naglalabas ng insulin ang lapay(pancreas)
 Araw araw na iniksyon ng insulin upang mabuhay

Ika-2 klaseng diabetes (Type 2- Non – insulin dependent)


 diabetes sa pagtanda
 ang katawan ay hindi nakakagawa ng sapat na insulin o hindi mabisang
nagagamit ang ginagawa nitong insulin
 nabubuo sa mga may sapat na gulang sa edad na 40

DIABETES SA PAGBUBUNTIS
 nabubuo o natutuklasan kapag nagdadalantao ang isang babae
 Karaniwang nawawala pgkatapos ng pagbubuntis
 Mas malaki ang tsansa na mgkaroon ng type 2 diabetes

 Karaniwang namamana
 Ang orihinal na sanhi ay hindi pa nalalaman ng mga eksperto

SANHI NG DIYABETIS TYPE 1

 Karaniwang namamana
 Ang orihinal na sanhi ay hindi pa nalalaman ng mga eksperto

SANHI NG DIYABETIS TYPE 2

 Sobrang katabaan o overweight


 namamana
 May high blood
 Kung ikaw ay nagkarron ng diyabetis habang nagbubuntis
 Pagkain ng mga makolesterol na pagkain o mataas n kolesterol sa katawan
 Mga taong tamad o kulang sa exercise

iv. SINTOMAS NG DIYABETES


 Laging pagkauhaw
 Laging pagihi
 Laging gutom
 Panlalabo ng paningin
 Biglang pagbagsak ng timbang
 Laging pagod
 Walang nararamdaman sa paa
 Hindi o matagal gumaling na mga sugat
 Madalas na impeksyon

v. PAGSUSURI NG DIYABETES
1. A1C
- Sinusuri ang blood glucose sa katawan ng 2-3 buwan ng nakaraan
2. FBS – Fasting Blood Sugar
- kailangan hindi kumain ng walong (8) oras bago ang test
- >126mg/dl = Diabetes
3. OGTT (Oral Glucose Tolerance Test)
4. Random Blood Glucose Test
- Pwede itong gwin kahit anong oras

vi. MGA PARAAN PARA MAIWASAN ANG DIYABETES


 Pagpapababa ng Timbang
 Kumain nang sapat na dami ng pagkain
 Iwasan ang pagkain nang maraming matatamis

vii. PANGANGALAGA SA TAONG MY DIABETES


 TYPE 1
o Araw araw na iniksyon ng insulin
o Tamang diyeta
o Ehersisyo
o Pag examine sa antas ng asukal sa dugo
 TYPE 2
o Magpakonsulta sa doctor kung ano ang kinakailangang inumin na
mga gamot para sa Diabetes
o Mag exercise at magbawas ng timbang
o Iniksyon ng insulin
o Bawasan o kaya tigilan ang mga nakaugalian bisyo
o Ang inyong duktor ang magpapayo ng tamang target na antas ng
asukal sa dugo.
o Kailangan ng regular na konsultasyon at pagmamatyag.

viii. KAHALAGAHAN NG MAY ALAM SA DIYABETES


Ang mga taong may diabetes ay:

 Mas mataas ang panganib ng magkaroon ng sakit sa puso


 Mas madalas na magkaroon ng atake sa utak (stroke)
 Mataas na panganib na magbara ang mga ugat sa kamay at paa
 Mas maagang namamatay

ix. KOMPLIKASYON NG DIYABETIS


 Panlalabo ng mga mata
 Pagkasira ng bato
 Pananakit o Pamamanhid ng ugat

Tandaan!

Ang diabetes ay hindi nawawala o gumagaling.


Maaaring mabawasan ang mga komplikasyon kung ang asukal sa dugo ay
makokontrol.

Palliative care meets the needs of all patients requiring relief from symptoms, and the needs of patients
and their families for psychosocial and supportive care. This is particularly true when patients are in
