4. 3R size, whole bodypicture of senior citizen and representative holding the latest newspaper

NOTE:
Social Pension for Indigent Senior Citizens is a program of the National Government
under the Department of Social Welfare and Development (DSWD).
OSCA is in charge of the RECRUITMENT, INTERVIEW, and VALIDATION of
eligibleapplicants based on the following criteria:
1. Senior Citizen aged 60 years old and above , indigent, sick and frail
2. No insurance from GSIS/ SSS/ PVAO
3. No financial support from relatives.

REQUIRED DOCUMENT/S − Voter’s ID, SSS, GSIS, Driver’s License, Postal ID, Police Clearance, and NBI,
(not expired) with address in Caloocan City
− Billing Address in Caloocan City: MERALCO, MAYNILAD, or Any Statement of
Account (SOA)
For lost Senior Citizen’s Identification Card:
1. Affidavit of Lost
2. One piece of 1x1 latest photo with white background
3. Any government-issued Identification Card or any billing addressed in Caloocan City
Senior Citizens with No Record at requirement b:
1. Birth Certificate of one of the children with the senior citizens’ age
2. Home visitation is required.
For Representative of Senior Citizen:
1. Authorization Letter

3. Whole body picture (3R) of senior citizen together with the representative holding
latest newspaper

All discrepancies in the above mentioned requirements must be supported by an Affidavit.
