3. Drug Test Clearance and Biometrics (PHP 500)
4. Gun Safety & Responsible Gun Ownership Certificate (P1,100)
5. Directorate for Intelligence (DI) / Regional Intelligence Division (RID)
Clearance  (PHP 80)
6. Regional Trial Court (RTC) & Municipal Trial Court (MTC) / Sandiganbayan
Clearance
7. National Statistics Office (NSO) Birth Certificate or Valid Passport
8. Government issued IDs (2)
9. Proof of Latest Billing / Certification of residency from Brgy Chairman
10.Valid ID
 
Additional Requirements:
 
For Private Employee
- Original & Photocopy of ITR / Employment Certificate (Indicate Company,
Position, Salary and Date Hired)
 
For Businessman/Businesswoman
- Business Permit, SEC, DTI or CDA Registration
 
For Retired Employee
- Retirement order or any appropriate document
 
For Farmers
- Land CTC
 
For Government Employee
- Appointment Order or Oath of Office
- Valid ID (Unit or Office ID)
