DEED OF UNDERTAKING

I, ____________, Filipino, of legal age and residing at ____________after being

sworn to in accordance with law, depose and say:

1. That I am the Chairperson, Transportation Service Sector of the

___________________(the “Cooperative”), a duly registered transport service
cooperative, pursuant to existing laws;

2. That I am the duly authorized representative of the representative and signatory of

the Petition, and in all other papers and documents, pertaining to the
Cooperative’s application for Special Permit pursuant to MC No. 2020-012 at the
LTFRB Cebu Regional Office;

3. That the Cooperative undertakes to produce and deliver the required number of
OFG-compliant modern buses, based on the following delivery schedule as
confirmed with the manufacturer/supplier reckoned from the date of submission
of pre-qualification documents, to wit:

25% of required number of units Within three (3) months

50% of required number of units Within six (6) months
100% of required number of units Within nine (9) months

4. That the Cooperative undertakes that the units shall be compliant with the
specifications and standards set under the OFG and the DTI-Philippine National
Standards (PNS);

5. That if the above-mentioned routes being applied for are not yet covered by Local
Public Transport Route Plan (LPTRP) or DOTr rationalization study, the
Cooperative hereby undertakes to fully comply with the approved rationalization
study;

6. That the Cooperative endeavors to acquire full ownership of the modern units.
However, should the Cooperative opt for the Lease (or Lease-to-Own) of units in
order to fully comply with the delivery of the required number of units when
unfavorable circumstances arise making it difficult to acquire immediate full
ownership, the Cooperative undertakes to fully comply with Board Memorandum
Circular No. 2018-015 pertaining to the acceptance of units under the Lease /
Lease to Own arrangement;

7. That the Cooperative undertakes to submit a compliant garage with zoning clearance
and compliant to the standards and specifications provided under Board
Memorandum Circular No. 2017-027, subject to verification and inspection,

 immediately upon availability of compliant PUV units, which is to be submitted on
or before the issuance of Provisional Authority;

8. That the Cooperative undertakes to provide copies of Letter of Support from property
owners/managers regarding access to, or use, each off-street loading and
unloading area in the route map;

9. That the Cooperative undertakes to submit valid Passenger Personal Accident

Insurance Policy (PPAIP) if the units are for Hire, once selected.

10. That the Cooperative undertakes to submit insurance benefits as that of PPAIP if the
units are private, either applicant owned or leased once selected.

11. That the Cooperative undertakes to follow specified bus marking guidelines under MC
numbers 2019-055, and 2020-009 once granted with Special Permit.

12. That I attest to the truthfulness of the foregoing undertaking.
