Updated quarterly matrix on the issuance of decisions by the Cluster Directors; 2. Updated quarterly matrix on reviewed and submitted Cash Examination Reports with discovered shortages to the Office of the Ombudsman; and 3. Updated quarterly matrix on the review and issuance of Audit Observations , Memoranda (AOMs) on novel and controversial issues. For monitoring purposes, the data needed by this Office for Items 1 and 2 can be obtained from the Cluster’s Quarterly Accomplishment Report. For Item 3, we request that the template below be accomplished.
