KNOW ALL MEN BY THESE PRESENTS:

I, JOSE CANERO TACDERAS of legal age, Filipino, single, and a resident of Maoasoas Sur , Pugo La Union,
after having been sworn in accordance with law, hereby deposes and states the following:

1. That I was retired member of the Armed Forces of the Philippines;

2. That I was issued an identification card by the Air Material Wing Savings and Loan Association,
Inc (AMWSLAI);
3. That the abovementioned Identification card was being used by me to avail of my pension and
obtain loans from AMWSLAI;
4. That when I was about to withdraw my loan from the said Corporation (AMWSLAI), I was not
able to find and locate such Id ;
5. That the said AMWSLAI Id was irretrievably lost and can no longer be found despite diligent
efforts to locate the same;
6. I execute this affidavit to be able to withdraw my loan from AMWSLAI and to attest to the truth
of the foregoing.

SUBSCRIBED and SWORN TO before me this 13th day of October. The affiant exhibited to me his
Philippine Veterans Affairs Id issued by the Department of National Defense which expires on
June 14, 2023 as competent proof of his identity.
