AFFIDAVIT OF SUPPORT.

I, BERYL ANYANGO AOKO, holder ID/No. 32919963 of KANYASREGA Sub-location,

North Sakwa – Location in Migori County within the Republic of Kenya, do hereby
make an oath and state as follows: -

1. THAT: I am a Kenyan female adult of sound mind domiciled at the above

address hence bearing a virtue of competence and capability of swearing
this Affidavit.

2. THAT: On attaining majority age, I applied for National Identity Card

aforementioned with the Registrar of Persons whence I embraced my
names as BERYL ANYANGO AOKO last name AOKO being my father’s name.

3. THAT: I am cognizant of the fact that as at the time of issuing of the certificate by the
Ministry of Education my name AOKO was erroneously entered in the said
document ( Original Teachers certificate in Early Childhood Education ) and
thus it bears ADHIAMBO SARAH OKUU instead of ADHIAMBO SARAH
OKUMU as per my other documents i.e. KENYA CERTIFICATE OF
SECONDARY EDUCATION CERTIFICATE ( KCSE) and NATIONAL
IDENTITY CARD which inadvertent mistakes I now wish to have corrected to
reflect what is in my academic documents. The subject matter of this affidavit.

4. THAT Due to matters aforementioned I make this declaration knowingly and

consciously and in accordance with OATH AND STATUTORY
DECLARATION ACT in support of my application to application to the
Ministry of Education to correct my name to bear ADHIAMBO SARAH
OKUMU and delete or/and exclude the names OKUU and /or replace it
with OKUMU.
5. THAT I further swear this affidavit to confirm that my true names are ADHIAMBO
SARAH OKUMU

6. THAT All the facts deponed herein are true to the best of my knowledge,

2. THAT: I was born in the Year 1944 or there about, by Mr. MICHAEL OKIRI GOR and
Mrs. JUSTIN OILE GOR both of KADEM NORTH Sub-location, SOUTH
KADEM Location in Migori District.

3. THAT: On attaining majority age, I applied for National Identity Card

aforementioned with the Registrar of Persons whence I embraced my
maiden names and thus my National Identity Card aforementioned bears
the names MARGARET ATIENO GOR the first name MARGARET, being my
maiden name and the last name GOR, being my father’s name.

4. THAT: I am equally cognizant of the fact that in the year 1980 or thereabout I
got married according to Luo Customary Laws and Rituals to JOHN
ATIANG AWINO, therefore I embraced my marital name ATIANG in
addition in my father’s name GOR and I have been using the two sets of
names interchangeably. The subject matter of this affidavit.

5. THAT Due to matters aforementioned I make this declaration knowingly and

consciously and in accordance with OATH AND STATUTORY
DECLARATION ACT in support of my application to application to the
Registrar of Persons to correct and /or change my name to bear
MARGARET ATIENO ATIANG.

6. THAT I further swear this affidavit to confirm that my true and / or correct
names are MARGARET ATIENO ATIANG.

1. THAT: I am a male adult of sound mind domiciled at the above address

hence bearing a virtue of competence and capability of swearing
this Affidavit.

2. THAT: My wife AGNES KWAMBOKA ONDERI of ID/No.23386362 was

born on 27th April, 1983 at Nyabera Sub-location by Mrs.JANET
ASIGO and the late Mr.NASHON ONDERI in Kisii County.

3. THAT: I swear that AGNES KWAMBOKA ONDERI is my only wife.

4. THAT: Since she is my wife, I have no objection on her inheriting my

surname OCHIENG and to be known as AGNES KWAMBOKA
OCHIENG.

5. THAT Consequently, I swear this affidavit in my support of application and

request the Register of Persons to issue her with new National
Identity Card bearing AGNES KWAMBOKA OCHIENG as her name

1. THAT: I am a Kenyan male adult of sound mind domiciled at the above address
hence bearing a virtue of competence and capability of swearing this
Affidavit.
2. THAT: I was born on 19th September, 1985 by Mr. PHILIP AWENE OTIENO
and Mrs. PAMELA AGOLA AWUOR, both of KANYAGWALA Sub-location,
CENTRAL SAKWA Location, in Migori District.

3. THAT: On attaining majority age, I applied for National Identity Card

aforementioned with the Registrar of Persons whence I embraced my
maiden names and thus my National Identity Card aforementioned bears
the names PHILIP OMONDI OTIENO the first name PHILIP being my maiden
name.
4. THAT: I am cognizant of the fact that as at the time of the said registration my last
name OTIENO was erroneously entered in my National Identity Card
aforementioned instead of AWENE as per my birth certificate, which
inadvertent mistakes I now wish to have corrected. The subject matter of this
affidavit.

5. THAT: I am equally cognizant of the fact that during my registration to the Kenya
Certificate of Secondary Education (KCSE), my name was entered as
OMONDI PHILIP AWENE, the last AWENE being my father’s name and I
have been using the two sets of names interchangeably. Also the subject
matter of this affidavit.

6. THAT Due to matters aforementioned I make this declaration knowingly and

consciously and in accordance with OATH AND STATUTORY
DECLARATION ACT in support of my application to the Registrar of
Persons to correct and / or change my name to bear PHILIP OMONDI
AWENE as per my Birth certificate and my other documents and delete
or/and exclude the name OTIENO and /or replace it with AWENE.

7. THAT I further swear this affidavit to confirm that my true and /or correct names
are PHILIP OMONDI AWENE.

8. THAT All the facts deponed herein are true to the best of my knowledge,
information and belief.
