AFFIDAVIT OF CHANGE OF NAME

I, OJO TIMILEYIN ELIZABETH, adult, Female, Fashion designer, Christian, citizen of

the Federal Republic of Nigeria, of Zone 5 No 16 Alafia Estate Alaka Ojoo , Ibadan ,
Akinyele Local Government Area in Oyo State, do hereby make oath and declare
as follows:
1. That I am the deponent herein and make this affidavit in good faith.
2. That I was formerly known, called and addressed as MISS ITIOLA TIMILEYIN
ELIZABETH.
3. That I now wish to be known, called and addressed as MRS OJO TIMILEYIN
ELIZABETH.
4. That the change of name was necessitated by my marriage to MR OJO
SUNDAY OLUWATOSIN. of Zone 5 No 16 Alafia Estate Alaka lbadan on the 13th
Day of April, 2024
5. That all documents bearing my former name(s) remain valid.
6. That this Affidavit is now made for record and official purposes.

That I swear to this affidavit in good faith believing same to be true and correct in
accordance with the Statutory Declarations Law, Cap 158, Volume V, Laws of Oyo
State of Nigeria, 2000.
