AFFIDAVIT OF BUSINESS CLOSURE

I, LEONOEL V. LOCQUIAO, Filipino, of legal age, married and a resident of 3317

Kaimito St.NAPICO, Bgy. Manggahan, Pasig City, Metro Manila, after having been duly sworn
in accordance with law do hereby depose and state:

That I am the Owner of the business name LECATRON Industrial Sales & Services
which is registered with the BPLO 4 of Pasig City, with business ID number PM- 2005-009041.

That I am applying for the termination of the business, LECATRON Industrial Sales &
Services, registered with DTI, and since it’s registration, the business didn’t perform well. In fact
it just lasted 5 months.

That the reason for the termination is due to mismanagement of the above mentioned
business.

The said establishment has ceased operations in good faith, effective May 19, 2005, and
at present has no pending debt, liability or obligations from any party, including the government,
local or national, SSS Pag-Ibig or PhilHealth.

That all the documents after its shut down was in possession of my then business partner
Catherine Jimenez and on September 28, 2009 those documents were destroyed as her house was
badly hit by flood due to typhoon Ondoy.

That I am executing this affidavit to attest to the truth of the foregoing statements and for
the purpose of complying with the government requirements on the matter.
