Preliminary Details of Incident


An employee was disassembling a Two-part tension pully from an auger drive. The pully was placed in a vice. The
tension bolt was removed completely to relieve any tension on the spring. The snap ring was removed from the end
cap. After the snap ring was removed, the employee started to open the vice.

What Went Wrong?


When opening the vice, the spring fully released
and flew about 20 feet Because the employee used
his experience with other equipment, he
positioned his body out of the line of fire and no
one was injured.

Immediate Cause(s)-
1. The task is not a routine task, it has never been performed at this location.
2. The employee is experienced with working on various types of oilfield equipment but not disassembling this
pully. He took steps to reduce the risk of spring tension but because of the design of the pully he did not realize
the amount of tension still on the spring.
3. No JHA was completed before doing this task.

Immediate Actions-

1. The disassembled pully has been removed from service and will be replaced with a new one.

2. Inform all area managers of the incident. Discuss that two-part tension pullies will not be disassembled at NOV
locations. They will be replaced with new ones or sent to the manufacturer for repairs.

3. Distribute this HSE alert to all locations.

Lessons Learned-
Before performing a task that has never been performed at a location a risk assessment must be performed. Consider if
employees are knowledgeable and qualified to perform the task. Also consider if special tools or equipment are
required to safely perform the task. Employees will not perform a task unless they are knowledgeable, qualified for the
task, and have the proper tools required.
