Preliminary Details of Incident


Employee injured finger when rolling pipe.
The IP was manually handling (rolling) 9.5” drill collars within the drill pipe inspection center (DPIC) production line.
Whilst attempting to record the serial number the IP reached down to the drill collar and tried to manually stop the drill
collar from rolling on the line.
What Went Wrong?
The IP was unable to stop the momentum gained within the drill collar and the IP’s hand was drawn into the drill collars
rotation resulting in one finger being trapped between the rolling drill collar and another stationary drill collar. The IP
was removed to hospital following the incident and sustained bruising and swelling to the tip of one finger.

Immediate Cause(s)-
The IP should not have been manually handling rotating pipes in this manner as all personnel are aware of the tool that
should be used to complete this task.

Immediate Actions-
Personnel to receive further instruction and training on safe working whilst completing this task.
Risk assessment to be updated to include the use of the tool.

Lessons Learned-

A tool exists for the manual rotation of pipes during this task; however, this was not used by the IP to incorrectly save
time and effort.
